卡泊芬净用于肺孢子菌肺炎的临床研究进展

肺孢子菌肺炎是一种严重的机会性肺部感染疾病，目前治疗的一线方案为复方磺胺甲噁唑，但是该药物容易引起严重的不良反应。由于肺孢子菌核苷酸序列与真菌之间具有高度相似性，大部分学者将它从原虫归属为真菌。β-葡聚糖是构成肺孢子包囊细胞壁的主要成分，在对宿主细胞的炎症反应中起到重要作用。因此，针对β-（1，3）-D-葡聚糖合成的卡泊芬净成为肺孢子菌肺炎治疗新的替代药物。尤其是卡泊芬净与复方磺胺甲噁唑联合使用的方案可能比一线方案具有更优的疗效和更低的不良反应发生率。本文就肺孢子菌的真菌特点、卡泊芬净用于肺孢子菌肺炎的临床研究进展作一综述。

Research advancement of caspofungin for Pneumocystis jiroveci pneumonia

Pneumocystis jiroveci pneumonia is a serious opportunistic pulmonary infection disease. Its first line treatment is compound sulfamethoxazole, which tends to cause serious adverse reactions. Due to the high similarity of the nucleotide sequence between Pneumocystis jiroveci and the fungus, it is classfied as fungi instead of protozoa by most scholars. β-glucan is a major component of the cell wall of the Pneumocystis jiroveci, and plays an important role in the inflammatory response to the host cells. Therefore, caspofungin targeting β-(1,3)-D-glucan has become a new alternative drug for,China pneumonia. In particular, the combination of caspofungin and compound sulfamethoxazole may have superior efficacy and lower incidence of adverse reactions than the first line regimen. Our study reviewed the fungal characteristics of Pneumocystis and the clinical research progress of caspofungin for, China pneumonia.
