光动力疗法治疗进展期食管癌的近期疗效观察

目的观察光动力治疗不能手术切除的进展期食管癌的近期疗效、不良反应及并发症。方法对17例临床Ⅳ期食管癌患者行内镜下光动力治疗,于治疗前及治疗后1、2、3、6及12个月复查内镜,并进行临床症状评定,随访生存时间,观察不良反应及并发症。结果17例患者中位随访时间7.5个月（3～12个月）;临床症状缓解率88.2%（15/17）;内镜下观察总有效率为64.7%（11/17）;半年生存率64.7%（11/17）;1年生存率41.2%（7/17）;未出现上消化道出血、食管气管瘘或食管纵隔瘘等严重并发症。结论光动力疗法可改善一部分进展期食管癌患者临床症状,使部分病例肿瘤缩小,其毒副反应较轻,是一种较好的晚期食管癌姑息治疗手段。

Observation on Effect of Photodynamic Therapy for Unresectable Advanced Esophageal Cancer

Objective：To evaluate the clinical efficiency and adverse effects ofphotodynamic therapy in unresectable advanced esoplaageal cancer. Methods： Seventeen patients with advanced esophageal cancer in stage IVtmderwent PDT. Treatment response of the tumors were determined by follow-up endoscopies 1,2,3,6,9 and 12 months after PDT. Symptomatic improvement of the patients and side effects of the photo sensitizer were recorded. Survival data was obtained in all the patients. Results： the median of follow-up period for the 17 cases was 7.5 months （3-12 months）. Symptomatic palliation was observed in 88.2% of the patients （15/17）. The total effective rate of endoscopy was 64.7% （11/17）. The survival rates of half a year and one year for the patients were 64.7% （11/17） and 41.2% （7/17）. There were not serious complications such as esophagorrhagia, esophagotracheal fistula and esophagomediasinal fistula. Conclusion： PDT is effective and safe in the treatment of some ofunresectable advanced esophageal cancer.
