老年糖尿病药源性低血糖50例临床分析

目的：探讨老年糖尿病患者药物治疗诱发低血糖的危险因素。方法：回顾和总结1994年1月--2002年6月的50例老年糖尿病药源性低血糖症，分析探讨老年糖尿病治疗中应注意的事项。结果：50例老年糖尿病药源性低血糖患者．伴有肾功能不全14例(28％)，高血压20例(40％)，冠心病10例(20％)，脑卒中12例(24％)。合并急性心肌梗死、心力衰竭l例，胃炎4例，肠炎l例，呼吸道感染7例，肝脓肿l例，足趾外伤并感染l例。使用磺脲类药物所致18例，磺脲类加双胍类药物所致23例，单用双胍类药物l例，胰岛素所致8例。结论：老年糖尿病患者在使用降糖药治疗过程中，药物过量或使用不当、肾功能不全、并发症全、多病同治、食物摄入不足等是引起低血糖症的危险因素。

Clinical analysis of drug induced hypoglycemia in 50 cases with senile diabetes.

Objective: To study the risk factors of hypoglycemia induced by medication in senile patients with dia-betes. Methods: 50 cases of senile diabetes with drug induced hypoglycemia from 1994. l to 2002.6 were reviewed and summa-rized, the matters needing attention during the treatmenl were analysed. Results: in 50 cases of senile diabetes with drug induced hypoglycemia, 14 cases(28%) were renal insufficiency, 20 cases(40%) were hypertension, 10 cases(20%) were coronary heart disease, 12 cases(24% ) were brain vessel accident. l case had acute myocardiac infarction and heart failure, 4 cases had gastritis, l case had enteritis, 7 cases had respiratory tract infection, l case had liver abscess, l case had toes trauma with in-fection. 18 cases were caused by sulfonylurea agent,23 cases were caused by sulfonylurea agent and biguanide agent, l case was caused by biguanide agent, 8 cases were caused by insulin. Conclusion: When the senile diabetis are treated with hypoglycemic agents, overdose or use inaptitude,renal insufficiency ,many complications, multidisease cured meantime.pooringestion and so on are the risk factors for causing hypoglycemia.
