宫颈疾病282例液基细胞学与病理学检查结果对照分析

目的评价液基细胞学诊断结果与病理检查结果的阳性符合率及其在宫颈良恶性病变诊断中的应用价值。方法对液基细胞涂片诊断阳性(ASC以上)并追踪到确切组织学诊断的282例患者的细胞学与组织学诊断结果进行对照分析。结果282例中细胞学诊断癌35例,HSIL44例,LSIL84例,ASC99例,AGC20例;与组织学结果对照,阳性符合率分别为:癌90.2%,HSIL83.0%,LSIL71.8%。11例ASCH中3例HSIL,3例LSIL;20例AGC中腺癌2例,鳞癌1例;CINⅢ级累及腺体2例,CINⅠ级4例,内膜增生过长4例。结论液基细胞学有助于癌、宫颈上皮内瘤变的检出及细胞学诊断阳性符合率的提高;2001版TBS系统对ASC的分类有助于临床医师对患者的管理;AGC患者宫颈和颈管细胞学检查阴性时,内膜诊刮是必要的。

Contrast analysis of liquid-based cytology and pathology for 282 cases of cervial diseases

Objective To evaluate the efficacy of liquid-based cytology(TCT).Methods TCT diagnosis of 282 cases with abnormal cervical cytologic results was compared with histopathology diagnosis.Results TCT diagnosis in 282 cases included 35 cases of cancer,44 cases of HSIL,84 cases of LSIL,99 cases of ASC and 20 cases of AGC.The conformity rate of TCT and histopathology diagnosis was 90.2% in cancer,83.0% in HSIL,71.8% in LSIL.Conclusion TCT is benefit to screening of cancer and CIN,and improve the conforming to histopathologic.New classification of ASC recommended by 2001 TBS has apparent clinical significance.
