分期针刺对老年偏瘫患者康复的疗效观察

目的探讨分期针刺配合康复训练对老年偏瘫患者康复的疗效观察，寻求进一步改善老年偏瘫患者运动功能、日常生活质量及对康复治疗认知度的临床效果。方法选择2010年12月-2012年6月门诊或住院的脑卒中患者100例。随机分为治疗组与康复对照组，每组50例。对照组仅行基础康复训练，治疗组在对照组的基础上再加分期针刺治疗，观察各组患者的入院及治疗前后的运动功能及日常生活活动能力。结果患者的一般基线情况具有可比性，治疗前各项评分两组组间没有明显差异；治疗后治疗组与对照组FMA评分分别为（62．35±17．29）和（53．11±20．22），MBI评分分别为（93．34±15．31）和（64．55±18．23），差异比较有统计学意义（均P〈0．05）；治疗后各组组内前后比较差异具有统计学意义（均P〈0．05）。结论现代康复理论指导下的分期针刺结合康复训练能更有效地改善老年脑卒中偏瘫功能障碍。

Clinical effects of staging acupuncture with rehabilitation therapy on elder patients with hemiplegia

Objective This study was designed to assess the effects of staging acupuncture with rehabilitation therapy on elder patients with hemiplegia. Methods Acupuncture stimulating points based on Brunnstrom stage combined with regu- lar physical therapies （PT） was used in the treating group, and only PT was used in the control group. All the cases were randomly assigned into these two groups （50： 50）. Cases in the treating group were administrated acupuncture for 30min per day and PT 45min per day, 5 times per week for totally 8 weeks. All were evaluated with Fugl-Meyer assessment scale （FMA） and Modified Barthel index （MBI） before and after treatment. Results There was no significant difference between AT group and BT group about the baseline at admission. There were significant differences in FMA （62.35±17.29 vs. 53.11 ± 20.22, P 〈 0.05 ） and MBI （ 93.34 ± 15.31 vs. 64.55±18.23, P 〈 0.05 ） between the two groups after intervention and significant differences were observed within the two groups. Conclusion Staging acupuncture under the guidance of modern rehabilitation may improve post-stroke hemiplegic dysfunction in elder patients more effectively.
