脊髓型颈椎病突发性截瘫的临床分析(附15例报告)

目的:通过对脊髓型颈椎病(CSM)突发性截瘫这一临床现象进行综合临床研究,探讨其发病机制和治疗措施.方法:研究15例CSM突发性截瘫患者的临床资料,总结出本组患者在临床表现、体征、影像学表现等方面不同于一般颈椎病患者的特点.结果:CSM突发性截瘫的患者首先表现出不同程度的脊髓压迫症;影像学表现为多节段椎管狭窄同时伴有单或多节段的不稳或滑脱,该多节段的脊髓压迫是产生脊髓严重损害的临界状态.结论:CSM突发性截瘫患者的病理基础是多节段的椎管狭窄同时伴有单或多节段的不稳或椎间盘突出.该病应尽早诊断;一旦发病,需立即手术.

Clinical analysis of flare paraplegia of cervical spondylotic myelopathy: A report of 15 cases

Objective:To study the pathogenesis and treatment of flare paraplegia of cervical spondylotic myelopathy(CSM). Methods:Clinical symptom,physical sign and imaging of 15 cases of CSM patients with flare paraplegia were studied, and the characteristic of flare paraplegia of CSM were summarized and compared with others. Results: Manifestation of flare paraplegia of CSM was firstly spinal cord compression.Manifestation of iconography consisted of multisection spinal canal stenosis and one or multisection protrusion of intervertebral disc and accompanied cervical instability.Conclusion: Intervertebral disc retrogress,cervical spinal stenosis and cervical instability is pathology bases of flare paraplegia of CSM,and it must be diagnosed and treated earlier.
