| 摘 要： | 自1978年3月至1992年4月共收治成人股骨头自发缺血坏死45例,以Mercus分法为Ⅲ—Ⅵ期。复查42例（48髋）。行五种术式治疗:（1）股骨头中心减压术7髋,以Jacobs评分平均手术前后由58.5分增至73.8分（P<0.05）。（2）股骨粗隆下旋转截骨术12髋,评分由49.5分增至88.8分（P<0.001）。（3）含旋髂深血管髂骨块移植术6髋,评分由39.3分增至69.8分（P<0.001）。（4）粗隆下旋转截骨术加含旋髂深血管髂骨块移植术20髋,评分由48分增至86.3分（P<0.001）。（5）坏死的股骨头软骨及软骨下坏死层切除,取肋软骨膜包裹股骨头3髋,效果欠满意。

| Abstract: | 42 cases (48 hips) of idiopathic osteonecrosis of the femoral head in adults were operated on randomly with five different procedures from May 1978 to April 1992. According to Mercus's stage,the pathology of these heads were belonged to III-VI stage. The patients were operated on and followed -up for an average of 49(10-123)months. According to Jacobs criteia.in group 1(7 hips treated by core-decompression) ,the average points increased from pre -operative 58. 5 to post-operative 63. 8;in group 2 ( 12 hips treated by subtrochanteric rotational osteotomy)from 49. 5 increased to 88. 8;in group 3(6 hips treated by vascular pedicled iliac bone grafting)from 39. 3 increased to 69. 8;in group 4(20 hips treated by method B and C combination)from 48 increased to 86. 3;in group 5,the results was unsatisfactory. |
