临床药师对头孢吡肟致尿毒症患者抗生素脑病的药学分析与监护

目的:提醒临床医师在使用头孢吡肟时加强对其神经毒性的监测,探讨临床药师在指导合理用药中的作用。方法:通过分析头孢吡肟的药动学特征、抗生素脑病症状出现的神经系统毒性表现及其诱发的作用机制,并根据现有不良反应报告,对该药的选用或替换提出合理用药建议,在脑病症状出现后及时提出行血液透析,缓解患者症状。结果:通过临床药师的建议,患者的感染症状迅速得到控制,脑病症状迅速得到缓解。结论:临床药师通过对特殊用药人群的药学监护,在合理用药和抗生素脑病治疗方面能够发挥重要作用。

Pharmaceutical Analysis and Care for Cefepime-caused Antibiotics Encephalopathy in Uremic Patient by Clinical Pharmacists

OBJECTIVE： To remind clinicians to strengthen the neurotoxicity monitoring of cefepime, and to investigate the role of clinical pharmacists in guidance for rational drug use. METHODS： By analyzing pharmacokinetic characteristics of ce- fepime, the symptoms of nervous system toxicity and mechanisms of antibiotics encephalopathy, and according to existing ADR re- ports, some recommendations about the choice and replacement of cefepime were put forward, and the hemodialysis were proposed timely after the onset of encephalopathy symptoms to alleviate the patient＇ s symptoms. RESULTS ： Through clinical pharmacist＇ s recommendations, the infection symptoms of patients quickly were put under control and the encephalopathy symptoms quickly were mitigated. CONCLUSIONS： Clinical pharmacists can play an important role in pharmaceutical care and antibiotics encepha- lopathy prevention and treatment by providing for speical drug use.
