超声引导穿刺活检在浅表肿大淋巴结良恶性诊断中的应用

目的通过超声引导活检技术的应用，比较三种针型（16G、18G、20G）穿刺活检的临床应用价值。方法应用美国Bard穿刺针经超声引导对125例肿大淋巴结患者穿刺活检，活检取出的组织能作出明确组织病理学诊断的为取材满意，以手术证实或随访观察6个月-1年的为最后诊断。结果125例中，116例取材满意，穿刺获得组织学诊断率92．8％，16G活检针活检44例，18G活检针活检54例，20G活检针活检27例，三种针型取材满意率间的比较及良恶性的诊断差异无显著性（P〉0．05）。20G针有1例假阴性，16G、18G针型无，三种针型间的比较无显著性差异（P〉0．05）。结论超声引导淋巴结穿刺活检技术方法简便，病理诊断阳性率高，对确定诊断有重要意义。

By the technology Us-guided biopsy study on diagnosis benign and maligant superficial tumescent lymph nodes

Objective Using the technology of Us-guided biopsy to initiate a method of pathologic diagnosis of early benign and malignant superficial tumescent lymph nodes. And to compare the clinical usefulness of 16G, 18G, 20G needle in biopsy. Methods Biopsy of lymph nodes was performed on 125 patients with American Bard needle of biopsy. A biopsy was considered successful if a specific bengn or malignant diagnosis was rendered by the pathologist. The final diagnosis was made by pathology diagnosis after operation or the result of follow up for 6 months to one year. Rresults There were 116 satisfied materials in 125 cases, from which defmite histologic diagnosis could be given ,reaching a 92.8% accuracy by biopsy. 16G was applied on 44 patients, 18G on 54 patients while 20G on 27 patients . There was no significant difference between the three types of needles in the eligibility of sampling. There was one false negative for 20G while none for 18G, 20G. There was no significant difference between the two types of needles. Conclusion For Us-guided percutaneons biopsy of lymph nodes. The method is handy and the accuracy of the pathologic diagnosis is high. So it is significant for definite histologic diagnosis.
