三维能量多普勒超声预测子痫前期的临床应用研究

目的探讨三维能量多普勒超声(3D-PDU)及其定量参数对预测子痫前期的应用价值。方法应用三维能量多普勒超声成像对328例11~14周进行常规超声检查的孕妇胎盘血流灌注情况进行前瞻性观察分析,应用三维多普勒直方图计算胎盘血管指数(VI)、血流指数(FI)、血管和血流指数(VFI)。所有患者在16~20周之间再进行一次检查。孕妇随访至分娩结束,按有无发生子痫前期分为正常妊娠组和子痫前期组。结果16~20周子痫前期组胎盘血管指数(VI)、血流指数(FI)、血管和血流指数(VFI)等胎盘血管指数明显低于对照组。11~14周子痫前期组胎盘血管指数与对照组差异无统计学意义。结论三维能量多普勒超声可定量分析胎盘血流灌注情况,可以预测及诊断子痫前期。

The clinical application of three-dimensional energy Doppler ultrasound in predicting preeclampsia

Objective To explore the value of 3D-PDU and its quantitative parameters in the prediction of preeclampsia.Methods The placental blood flow perfusion of 328 pregnant women who underwent routine ultrasound examination at 11~14 weeks was observed and studied prospectively.The placental blood vessel index(VI),blood flow index(FI),blood vessel and blood flow index(VFI)were calculated by three-dimensional Doppler histogram.All patients had another examination between 16 and 20 weeks.The pregnant women had been followed up to the end of delivery,and were divided into normal pregnancy group and preeclampsia group,according to result of preeclampsia diagnosis.Results The placental vascular index(VI),blood flow index(FI),blood vessel and blood flow index(VFI)of preeclampsia group in 16~20 weeks were significantly lower than those in the control group.There was no statistical difference in placental vascular index between the preeclampsia group and the control group at 11~14 weeks.Conclusion As a value tool to quantitatively analyze the placental blood perfusion,three dimensional power Doppler ultrasound can be used to predict and diagnose preeclampsia.
