The diagnostic value of DWI and conventional MRI features in testicular malignant tumor

Objective We aimed to explore whether preoperative diffusion-weighted magnetic rensonance imaging(DWI)and conventional MRI,so as to improve diagnostic efficiency of testicular tumors.Methods A total of 46 cases of malignant lesions included 25 cases of seminoma and 13 cases of nonseminomatous germ cell tumor(NSGCT),8 cases of lymphoma.All the 46 cases underwent conventional MRI and DWI.The correlation coefficient(ICC)and Kappa analysis were used to analyze the consistency of the two measurements.Ranked data were compared by Kruskal-Wallis rank sum test.One-way analysis of variance and least significant difference were used to analyze the difference of ADC value between different tumors.Results ADC values and T₂-hypeintensity,T₂-homogeneity,cystic or necrosis,intratuminal septum,and stage of primary tumor showed statistically significant differences among seminoma,NSGCT and lymphoma(P<0.05).T₂-hypeintensity,T₂-homogeneity were more common in seminoma and lymphoma than NSGCT.Cyst and necrosis were more common in NSGCT than in seminoma and lymphoma(P<0.05).Intratumoral septas were more common in seminoma than in NSGCT and lymphoma(P<0.05).The T stage of lymphoma was higher than that of seminoma and NSGCT(P<0.05).The ADC value of non seminoma was the highest,seminoma was the second,and lymphoma was the smallest.Conclusion The combination of DWI and conventional MRI features has a high differential diagnosis efficiency for subtypes of testicular malignant tumors.
