低分子肝素在过敏性紫癜中应用观察

目的探讨低分子肝素在过敏性紫癜中应用。方法选取2008-01-2012-05收治的符合入选标准过敏性紫癜儿童162例,分为低分子肝素组98例,常规治疗组64例,观察两组近期、远期疗效。结果低分子肝素组在近期疗效皮疹消退时间、消化道症状消失时间、关节症状消失时间上显著优于常规治疗组经统计学比较差异有统计学意义（P〈0.05）;在远期疗效中肾损害发生率显著低于常规治疗组,经统计学比较差异有统计学意义（P〈0.05）。结论过敏性紫癜是是一种IgA介导免疫变态反应性疾病,主要病理改变为小血管炎,急性期血小板聚集功能增强,形成高粘滞血症,血液浓缩,同时小血管炎造成毛细血管通透性增高,血流缓慢,高粘滞血症,红细胞聚集造成组织缺氧加重,免疫复合物沉着增加,促进红细胞聚集,导致炎症介导免疫反应加剧,进一步加重临床症状,并且大量的免疫复合物沉积是肾小球基底膜,造成肾脏损害。在过敏性紫癜中应用低分子肝素具有应用方便,良好的抗凝、保护血管内皮、防止阻断肾损害作用,减轻临床症状,缩短病程,减少紫癜性肾炎发生,

Low molecular weight heparin in anaphylactoid purpura observation

Objective Study of low molecular weight heparin on allergic purpura in application.Methods select 2008-01-2012-05 from met the inclusion criteria of Henoch-Schonlein purpura in children 162 cases,divided into low molecular weight heparin group 98 cases,conventional treatment group 64 cases,observation of two group in the near future,long-term effect.Results The low molecular heparin group in the near future curative effect rash subsided time,gastrointestinal symptoms disappear time,joint symptoms disappear time significantly superior to conventional therapy group was statistically significant difference of P 0.05;in the long-term curative effect of renal injury rate was significantly lower than the conventional treatment group,statistical comparison of P 0.05 there were significant differences.Conclusion Henoch-Schonlein purpura is a IgA mediated allergic disease,the main pathological changes of small vessel vasculitis,acute phase enhanced platelet aggregation,formation of hyperviscosity syndrome,blood concentration,at the same time,small vessel vasculitis caused the increase of capillary permeability,slow blood flow,hyperviscosity,red cell aggregation caused by tissue hypoxia is aggravating,immune complexes increased pigmentation,promote the aggregation of red blood cells,causing inflammation mediated immune reaction increased,further exacerbation of clinical symptoms,and a large number of immunologic match deposit is the glomerular basement membrane,causing kidney damage.In anaphylactoid purpura by low molecular weight heparin has convenient application,good anticoagulant,protecting vascular endothelium,prevent blocking the renal damage,relieve clinical symptoms,shorten the course of disease,reduce the occurrence of Henoch-Schonlein purpura nephritis,
