CT引导下介入治疗盘源性腰腿痛急性发作疗效分析

目的：评估CT引导下药物介入治疗盘源性腰腿痛急性发作近期和中期疗效，探讨影响疗效的相关因素。方法2010-05-2013-03对我院康复门诊58例盘源性腰腿痛急性发作患者行CT引导下药物介入治疗，疗效评价采用Macnab标准和汉化Oswestry功能障碍指数（ODI）。采用多元logistic回归分析患者年龄、靶层面突出分型、急慢性病程长短和介入次数等对疗效结局的影响。结果术后1月、6月、1年和2年随访与治疗前比较，患者改良Oswestry功能障碍指数各条目及ODI总分有逐渐降低的趋势，差异有统计学意义（P＜0.01）。患者年龄、靶层面突出分型、急慢性病程长短和介入次数等尚不构成影响介入预期疗效的独立危险因子（P＞0.05）。结论 CT引导下药物介入治疗是值得尝试和推荐的有效治疗手段，中期疗效稳定。

Evaluation of curative effect about CT-guided interventional injection treatment for acute discogenic lumbago-leg pain

Objective To evaluate the curative effect of CT-guided interventional injection treat-ment for acute discogenic lumbago-leg pain and analysize the prognostic factors. Methods Fifty-eight patients involved with discogenic lower back pain and radical leg pain were undergone CT-guided interventional injection treatment from May 2010 to March 2013. Macnab criteria and Chi-nese version of Oswestry low back pain disability questionnaire were used to assess the curative effect. The prognostic factors,such as age,herniation type of target segment,acute and chronic course time,and intervention times were testified by using multiple logistic regression analysis. Re-sults There were statistically decreased trend in ODI scores in the follow-ups at 1,6,12,24 months （P〈0.01）. There were not yet got statistically significant risk factors predicting clinical re-sults （P〉0.05）. conclusion CT-guided interventional injection was valuably recommended effective treatment for acute discogenic lumbago-leg pain,that it had got favorable mid-term outcomes.
