短期胰岛素强化治疗诱导初诊2型糖尿病患者血糖长期良好控制的临床研究

目的分析短期应用胰岛素进行强化治疗对于2型糖尿病（T2DM）患者的胰岛β细胞功能以及血糖控制水平的影响。方法收集初诊T2DM患者46例,以胰岛素泵进行治疗2周以后,测定患者治疗前后的血糖以及胰岛素变化情况,并计算患者的胰岛β细胞功能指数（HOMA-β）以及胰岛素抵抗指数（HOMA-IR）,并观察血糖控制情况。结果患者的胰岛素水平、HOMA-β均较治疗前明显提高,HOMA-β以及血糖水平均明显降低（P〈0.05）;16例患者仅依靠运动以及饮食控制便取得了理想的血糖控制效果。结论对初诊T2DM患者实施短期胰岛素泵治疗能够快速控制血糖水平,有效改善其胰岛β细胞功能,并可减轻患者的胰岛素抵抗,值得推广应用。

Clinical study on short-term intensive insulin therapy inducing long-term good glycemic control in newly diagnosed type 2 diabetic patients

Objective To analyze the influence of the short-term intensive insulin therapy on the pancreatic is- let β-cell function and the glycemic control level in the patients with type 2 diabetes mellitus（T2DM）. Methods 46 patients with newly diagnosed T2DM were collected. After 2-week insulin pump therapy,blood glucose and the insu- lin changes were measured before and after treatment and the pancreatic islet β-cell function index（HOMA-β） was calculated. The glycemic control condition was observed. Results The insulin levels and HOMA-β were significantly increased than before treatment, HOMA-β,and blood glucose level were significantly reduced（P〈0.05） ;16 cases a- chieved the ideal glycemic control effect by relying on the exercise and the diet control alone. Conclusion Implemen- ting the short-term insulin pump therapy in the newly diagnosed T2DM patients can quickly control the blood sugar levels and effectively improve the pancreatic islet β-cell function and reduce the patient＇s insulin resistance, which is worthy of being widely promoted and applied.
