超声对肝门部胆管癌术前分期诊断

目的 评价超声对肝门部胆管癌术前分期诊断价值。方法 对５６例经手术病上实的肝门部胆管癌进行超声研究，观察肿瘤对周围组织的浸润范围。肿瘤与门静脉的关系，以及肝内外转移和淋巴结转移情况。结果 超声对肝门部胆管 中块的检出率为９１％，对门静脉受侵，肝门部淋巴结，肝和胆囊浸润，腹膜转移的诊断符合率分别为９０％、３３％、３５％，０％。在行彩超检查的４７例患者中，肿瘤浸润肝或胆囊及脐内外转移者２４例，转移组采

Staging of Hilar Cholangiocarcinoma with Ultrasound

Purpose To evaluate staging of hilar cholangiocarcinoma by ultrasound. Methods Extraductal infiltration into the surrounding tissue and relationship between tumors and the portal vein were observed in 56 patients with ultrasound as well as metastases in regional lymph nodes,liver or gallbladder and peritoneum.All these cases were confirmed by operation and pathology. Results A tumor mass was shown in 51 of 56.Ultrasound was proved accurate in detecting the portal vein involvement in 10 of 11 and metastases in regional lymph nodes in 8 of 24,liver or gallbladder in 9 of 26 and peritoneum in 0 of 4.The rate of blood flow displaying in the tumor was higher in metastatic group than that in non metastatic group.The main reason for poor sensitivity in detecting infiltration of liver or gallbladder was discussed. Conclusion Ultrasound is valuable for predicting tumor extension.
