红细胞压积与冠状动脉病变的相关性研究

摘    要:目的 探讨红细胞压积(HCT)与冠心病的相关性.方法 282例患者根据冠状动脉造影结果分为冠心病组和非冠心病组,分别检测两组血常规、空腹血糖、总胆固醇、甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、血肌酐、左室射血分数等指标,计算冠状动脉Gensini积分.结果 冠心病组红细胞压积低于非冠心病组,差异有统计学意义(P〈0.01).以HCT的四等分计算冠状动脉评分均值,红细胞压积明显下降或增加,冠状动脉Gensini积分均增加,呈"U"形曲线;多因素Logistic回归分析显示性别、年龄、空腹血糖、血肌酐、冠状动脉积分、HCT、血红蛋白与冠心病密切相关(OR=0.76~3.02,P<0.05,<0.01).结论 红细胞压积与冠心病关系密切,红细胞压积过低或过高可能预示冠状动脉病变严重.

The Anlysis of Association between the Hematocrit Level and Coronary Heart Disease.

Abstract:Objective To investigate the association between the hematocrit (HCT) level and coronary heart disease (CHD). Meth- otis According to the results of coronary artery angiography, 282 cases were categorized into 2 groups: CHD group and non-CHD group. They were detected by routine blood (including red blood cell count, hemoglobin(HGB), hematocrit(HCT), rod cell distri- bution width(RDW), fasting blood glucose, total cholesterol(TC), triglycerides(TG), high-density lipoprotein cholesterol(HDL-E), low-density lipoprotein cholesterol(LDL-C), serum croatinine, and left ventricular ejection fraction(LVEF), then calculated coronary gensini scores. Results The hematocrit level( % ) in CHD group is significantly lower than which in the non-CHD group (37.88 ± 4.95 vs 40.15 ± 3.08, P = 0.001). The coronary gemini average scores calculated with four equal HCT. When hematocrit level de- creased or increased, the coronary gensini average scores increased with a "U" shaped curve(76.14± 40.83, 40.28 + 29.45, 29.06 ± 25.58, 47.47 ± 36.87) .The multivariate Logistic regression analysis showed that gender, age, fasting blood glucose, serum croa- tinine, coronary scores, HCT, HGB independently associated with coronary artery disease. Condusions The hematocrit level is closely related with coronary artery disease. The unnormal hematocrit level may indicate severe coronary artery lesions.
