宫腔镜下子宫粘连电切术后不同方法预防宫腔粘连的效果分析

目的比较宫腔镜下子宫粘连电切术后不同方法预防宫腔粘连的效果。方法对2018年1月~2019年1月于我院行宫腔镜下子宫粘连电切术治疗的80例患者进行研究,依据随机对照原则分为两组,每组40例。对照组患者接受雌孕激素人工周期+生物电刺激治疗,观察组患者接受雌孕激素人工周期+宫腔球囊治疗。比较两组患者治疗效果、手术前后子宫内膜厚度变化情况、治疗期间不良反应及术后6个月复查宫腔再粘连发生情况。结果观察组患者治疗总有效率高于对照组,组间比较差异显著(P0.05);观察组患者术后3个月、6个月子宫内膜厚度均高于对照组,组间对比差异显著(P0.05);两组患者治疗期间恶心、腹痛、阴道不规则出血发生率组间比较,差异无统计学意义(P0.05);观察组患者术后6个月复查宫腔再粘连发生率低于对照组,组间比较差异显著(P0.05)。结论宫腔镜下子宫粘连电切术后采用雌孕激素人工周期+宫腔球囊治疗能够提高治疗效果,促进患者子宫内膜修复,降低宫腔再粘连发生率,值得临床推荐。

Effects of different methods on preventing intrauterine adhesion after hysteroscopic uterine adhesion electrotomy

Objective To compare the effects of different methods on preventing intrauterine adhesion after hysteroscopic uterine adhesion electrotomy.Methods Eighty patients treated with hysteroscopic uterine adhesion electrotomy in our hospital from January 2018 to January 2019 were enrolled and divided into two groups according to the principle of randomized control,with 40 patients in each group.The control group was treated with estrogen and progesterone artificial cycle+bioelectric stimulation and the observation group was treated with estrogen and progesterone artificial cycle+intrauterine balloon.The therapeutic effects,changes of endometrial thickness before and after the surgery,adverse reactions during the treatment and occurrence of intrauterine re-adhesion 6 months after the surgery of the two groups were compared.Results The total effective rate of the observation group was higher than that of the control group,with significant between-group difference(P<0.05).The endometrial thickness of the observation group was higher than that of the control group both 3 and 6 months after the surgery,with significant between-group differences(P<0.05).There was no significant difference in the incidences of nausea,abdominal pain and irregular vaginal bleeding between the two groups(P>0.05).The incidence of intrauterine re-adhesion 6 months after the surgery of the observation group was lower than that of the control group,with significant between-group difference(P<0.05).Conclusion The application of estrogen and progesterone artificial cycle+intrauterine balloon treatment after hysteroscopic uterine adhesion electrotomy can improve the therapeutic effect,promote endometrial repair and reduce the incidence of intrauterine re-adhesion,which is worthy of clinical recommendation.
