ICU脓毒症患者认知功能障碍影响因素分析

摘    要:目的探讨ICU脓毒症患者认知功能障碍影响因素。方法选择2018年1~12月ICU脓毒症患者288例作为研究对象。采用蒙特利尔认知评估量表(Mo CA)对抢救成功者的认知功能状态进行评价,根据结果分为认知功能障碍组与无认知功能障碍组。单因素及多因素分析影响ICU脓毒症患者发生认知功能障碍的影响因素。结果单因素分析结果显示:发生低氧血症、脑梗死、高血糖的患者发生认知功能障碍率较高(P0.05);发生认知功能障碍的患者谵妄持续时间显著长于无认知功能障碍的患者,差异有统计学意义(P0.05)。多因素分析结果显示发生脑梗死、谵妄持续时间、低氧血症、血糖升高是ICU脓毒症患者认知功能障碍的独立危险因素(P0.05)。结论 ICU脓毒症患者会发生认知功能障碍,低氧血症、脑梗死、高血糖、谵妄持续时间等是导致认知功能障碍的高危因素。

Analysis of factors influencing cognitive dysfunction in patients with ICU sepsis
Abstract:Objective To investigate the influencing factors of cognitive dysfunction in patients with sepsis in ICU.Methods A total of 288 patients with ICU sepsis were selected from January to December 2018.The cognitive function status of successful rescuers was evaluated using the Montreal Cognitive Assessment Scale(MoCA).The patients were divided into cognitive dysfunction group and non-cognitive dysfunction group according to the results.Univariate and multivariate analysis was used to analyze factors affecting cognitive dysfunction in patients with sepsis in ICU.Results Univariate analysis showed that patients with hypoxemia,cerebral infarction and hyperglycemia had higher rate of cognitive dysfunction(P<0.05),patients with cognitive dysfunction had significantly longer duration of delirium than those with non-cognitive dysfunction.The difference was statistically significant in patients with dysfunction(P<0.05).Multivariate analysis showed that cerebral infarction,duration of delirium,hypoxemia,and elevated blood glucose were independent risk factors for cognitive dysfunction in ICU patients with sepsis(P<0.05).Conclusion Patients with sepsis in ICU may develop cognitive dysfunction.Hypoxemia,cerebral infarction,hyperglycemia and duration of delirium are the risk factors for cognitive dysfunction.
