血清同型半胱氨酸、胱抑素C及尿微量白蛋白联合检测在糖尿病早期肾病中的意义
摘    要:目的分析糖尿病早期肾病患者应用血清同型半胱氨酸以及胱抑素C、尿微量白蛋白联合检测的价值。方法选取2017年10月~2018年12月我院193例2型糖尿病患者,分为单一糖尿病组(n=74)、早期糖尿病肾病组(n=62)、临床糖尿病肾病组(n=57),并选取67例健康体检人士为对照组,对比四组各项结果。结果临床糖尿病肾病患者m ALB、Cys C以及Hcy水平均明显高于其他组,早期糖尿病肾病组患者mALB、Cys C以及Hcy水平均明显高于单一糖尿病组(P0.05)。糖尿病各组患者mALB、Cys C以及Hcy水平均明显高于对照组(P0.05)。临床糖尿病肾病组患者均明显高于其他各组(P0.05)。早期糖尿病肾病组患者各项指标均高于单一糖尿病组患者(P0.05),糖尿病各组患者阳性检出率均明显高于对照组(P0.05)。Hcy+Cys C、mALB+Cys C、Hcy+mALB联合检测阳性检出率均明显高于单项检测(P0.05),Hcy+Cys C+mALB阳性检出率明显高于单项检测以及联合检测,差异有统计学意义(P0.05)。单项检测特异度以及敏感度差异均无统计学意义(P0.05),联合检测敏感特异度与单项检测特异度差异无统计学意义(P0.05),联合检测敏感度明显高于单项检测,差异有统计学意义(P0.05)。结论糖尿病早期肾病患者应用mALB、Cys C以及Hcy联合检测能够使阳性检出率以及敏感度得到显著提高。

Significance of combined detection of serum homocysteine,cystatin C and urinary microalbumin in early diabetic nephropathy
Abstract:Objective To analyze the value of combined detection of serum homocysteine,cystatin C and urinary microalbumin in patients with early diabetic nephropathy.Methods A total of 193 patients with type 2 diabetes treated in our hospital from October 2017 to December 2018 were divided into the single diabetes group(n=74),the early diabetic nephropathy group(n=62)and the clinical diabetic nephropathy group(n=57).And sixty-seven healthy subjects were selected as the control group.The results of the four groups were compared.Results The mALB,Cys C and Hcy levels in the clinical diabetic nephropathy group were significantly higher than those in other groups,and the mALB,Cys C and Hcy levels in the early diabetic nephropathy group were significantly higher than those in the single diabetes group(P<0.05).The mALB,Cys C and Hcy levels in each group with diabetes were significantly higher than those in the control group(P<0.05).The clinical diabetic nephropathy group was significantly higher than the other groups(P<0.05).The early diabetic nephropathy group was significantly higher than the single diabetes group(P<0.05).The positive detection rate of each group with diabetes was significantly higher than that of the control group(P<0.05).The positive detection rates of Hcy+Cys C,mALB+Cys C and Hcy+mALB combined detection were significantly higher than the positive detection rate of the single detection(P<0.05).The positive detection rates of Hcy+Cys and C+mALB were significantly higher than those of the single detection and the combined detection,with statistically significant differences(P<0.05).There was no statistically significant difference in the specificity and sensitivity between single detections(P>0.05).There was no statistically significant difference in the specificity between combined detection and single detection(P>0.05).The sensitivity of combined detection was significantly higher than that of single detection,with statistically significant difference(P<0.05).Conclusion The application of combined detection of mALB,Cys C and Hcy in patients with early diabetic nephropathy can significantly improve the positive detection rate and sensitivity.
