不同机械通气模式治疗老年重症胸部损伤合并急性呼吸窘迫综合征的临床效果观察
摘    要:目的 讨论对老年重症胸部损伤合并急性呼吸窘迫综合征患者进行不同机械通气模式治疗的实际效果和应用价值。方法 选取本院2018年1~12月收治的老年重症胸部损伤合并急性呼吸窘迫综合征患者96例,根据入院先后将其均分为A、B两组,每组48例,A组采用同步间歇性指令通气治疗;B组采用连续性气道通气治疗。比较两组患者治疗前后血氧分压(PaO2)、每分钟通气量(MV)、氧饱和度(SaO2)、动脉血二氧化碳分压(PaCO2)和总有效率。结果 治疗前、治疗后PaO2、MV、PaO2和PaCO2比较中,A组与B组无差异(P0.05)。同组治疗前后PaO2、MV、SaO2和PaSO2比较,差异显著(P0.05)。A组和B组总有效率分别为93.75%和95.83%,两组患者总有效率无差异(P0.05)。结论 对老年重症胸部损伤合并急性呼吸窘迫综合征患者进行不同机械通气模式效果相当,可恢复正常呼吸功能,临床上可根据老年患者实际情况选择合适机械通气方式。

Clinical effects of different mechanical ventilation modes on elderly patients with severe chest injury and acute respiratory distress syndrome
Abstract:Objective To analyze and discuss the practical effects and application value of different mechanical ventilation modes in the treatment of elderly patients with severe chest injury and acute respiratory distress syndrome.Methods Ninety-six elderly patients with severe chest injury and acute respiratory distress syndrome of our hospital from January to December 2018 were enrolled and divided into group A and group B according to the admission order,with 48 patients in each group.Group A was treated with synchronized intermittent mandatory ventilation and group B was treated with continuous airway ventilation.The blood oxygen partial pressure (PaO2),minute ventilation (MV),oxygen saturation (SaO2),arterial blood carbon dioxide partial pressure (PaCO2) and total effective rates of the two groups before and after treatment were compared.Results In the comparison of PaO2,MV,SaO2and PaCO2before and after treatment,there was no difference between group A and group B (P>0.05);there was statistical significance in the comparison of PaO2,MV,SaO2and PaCO2before and after treatment in the same group (P<0.05).The total effective rates of group A and group B were 93.75% and 95.83%,respectively,with no significant difference between the two groups(P>0.05).Conclusion The effects of different mechanical ventilation modes on elderly patients with severe chest injury and acute respiratory distress syndrome are equivalent.The normal respiratory function is restored.Clinically,the appropriate mechanical ventilation mode can be selected according to the actual situation of elderly patients.
