早产胎膜早破106例临床分析

目的:探讨早产胎膜早破(PPROM)发生的危险因素、对母儿的危害及其防治。方法:回顾性分析106例早产胎膜早破的发生原因及对母婴的影响,并对不同孕龄组28~32+6周、33~34+6周、35~36+6周3组作分析比较。结果:胎位不正(臀位和横位)、多胎、早产流产史、妊高征占据PPROM病因的前4位。不同孕周的3组早产对母亲影响无显著性差异;在新生儿呼吸窘迫综合征(RDS)及新生儿病死率有显著性差异(P<0.01)。结论:早产并胎膜早破是新产儿发病和死亡的主要原因。胎龄越小、死亡率越高。提高早产并胎膜早破的预防、诊断及治疗水平,对提高产科质量有重要的意义。

Study on premature delivery & preterm premature rupture of membranes

Objective: To investigate the risk factors and its influence on infants and mothers and prevention and cure of premature delivery and preterm premature rupture of membranes(PPROM).Methods: Causes and influence on infants and mothers of 106 cases PPROM were analyzed retrospectively;datas among different groups of gestational age of 28～32+6 weeks,33～34+6 weeks and 35～36+6 weeks were compared.Results: The main causes of PPROM were abnormal fetal presentations,such as breech presentation,transverse presentation,multiplets,history of abortion and premature delivery,pregnancy-induced hypertension.The influences of premature delivery of different gestational age on the pregnant women had no significant differences.Of neonatal respiratory distress syndrome(RDS),neonatal mortality rate,there were significant differences among the different groups of gestational age(P<0.01).Conclusion: The main causes of neonatal ill and death are premature delivery & PPROM.The younger the gestational age,the higher the neonatal mortality rate.Raising the levels of prevention,diagnosis and treatment of premature delivery& PPROM is significant to the improvement of the quality of perinatal medicine.
