持续皮下注射胰岛素治疗糖尿病高血糖危象效果观察

目的 探讨持续皮下注射胰岛素治疗糖尿病高血糖危象的疗效和护理方法.方法 将53例因高血糖危象急诊住院治疗的糖尿病患者分为两组.泵组(20例)使用胰岛素泵持续皮下注射胰岛素;小剂量组(33例)静脉滴注小剂量胰岛素,血糖控制后改为胰岛素餐前皮下注射.连续10 d后评价效果.结果 两组均可迅速降低高血糖(均P＞0.05);泵组降糖更稳定(均P＜0.01),但低血糖发生率显著高于小剂量组(P＜0.05).结论 持续皮下注射胰岛素能迅速降低血糖,可使血糖维持在较稳定水平;充分补液和严密监测血糖变化可望降低低血糖发生率,提高治疗安全性.

Efficacy and Nursing of Continuous Subcutaneous Insulin Injection in the Treatment of Acute Hyperglycemia in Diabetic Patients

Objective To investigate the efficacy and nursing interventions of continuous subcutaneous insulin injection (CSII) in the treatment of acute hyperglycemia in diabetic patients.Methods Fifty-three hyperglycemia diabetic subjects admitted from Emergency Department were divided into two groups: group A (CSII group, 20 cases); group B (intravenous infusion of low dose insulin, then subcutaneous injection after controlling of blood glucose, 33 cases). The differences in blood glucose levels in the first 10 days were evaluated.Results Both group A and group B lowered blood glucose rapidly, and there were no significant differences between them (all P>0.05), while efficacy of group A is better (all P<0.01). The incidence of hypoglycemia was higher in group A than in group B (P<0.05).Conclusion CSII can release from the condition of acute hyperglycemia in diabetic patients as intravenous infusion of low dose insulin. And it can bring a better control of blood glucose. Sufficient fluid infusion and carefully monitoring can assure the efficacy in order to avoid hypoglycemia.
