蛛网膜下腔出血不同时期脑脊液细胞学检查的临床意义

摘    要:目的应用细胞玻片离心仪和MCDS-2011细胞诊断系统对17例蛛网膜下腔出血患者进行了不同时期脑脊液细胞学检查。方法常规抽取脑脊液,经Cytospin3型细胞玻片离心仪离心制片,进行MGG染色。用MCDS-2011细胞学诊断系统进行细胞分类。结果蛛网膜下腔出血后脑脊液中白细胞在不同时期的细胞特征是:4小时-3天的内脑脊液中是以嗜中性粒细胞反应为主,3天后明显减少。3—5天出现单核-吞噬细胞,并可见到红细胞吞噬细胞。5—7天脑脊液中开始出现含铁血黄素吞噬细胞。结论应用该法进行脑脊液细胞学检查,可给临床提供一个直观、准确的诊断依据。对蛛网膜下腔出血病人的诊断、病理学监测和治疗具有重要价值。

Abstract:ObjectiveThe cytological examination in different period of cerebrospinal fluid was performed by cell slice centrifugal instrument in 17 subarachnoid hemorrhages(SAH).MethodsThe cerebrospinal fluid was obtained by routine way,with cytospin-3 type cell slice centrifugal instrument and display cells on the screen by means of MCDS-2011 cell diagnosis system.ResultsCharacteristics of white blood cell at different times in cerebrospinal fluid in subarachnoid hemorrhages were: Mainly cerebrospinal fluid reaction with neutrophils within three-day and four hours,but there was a distinct decrease three days later.Single nucleus lick-up cells appear in three to five days and red blood cells and lick-up cells can be seen.Blood flavin lick-up cells,which contain iron,begin to appear in five to seven days.ConclusionA direct and exact diagnosis gist can be offered to clinic by means of the cytological examination of cerebrospinal fluid which has important value to the diagnose of the Subarachnoid hemorrhaged patients,to the inspection and treatment of pathology.
