数字化成像结合纤支镜选择性支气管造影并肺穿刺活检诊断肺部疾病

目的：探讨数字化成像结合纤支镜选择性支气管造影并肺穿刺活检对肺部疾病的诊断价值。材料与方法：对２３例疑肺部肿瘤等疾病患者，先行胸部正侧位片及ＣＴ扫描检查，其中部分做了支气管体层摄影，然后施行纤支镜检、刷检，并在电视透视下经纤支镜进行肺穿刺活检。再应用数字减影（ＤＳＡ），数字化快速摄影８帧／ｓ，激光照像技术，用７６％复方泛影葡胺经纤支镜进行选择性支气管造影。结果：本组全部病例均由这种综合性技术检查确诊，并得到手术病理证实。检出中央型肺癌１３例，周围型肺癌５例，细支气管肺泡癌１例，肺囊肿１例，支气管内膜结核并肺不张２例，炎性假瘤１例。结论：该技术能准确诊断多种肺部疾病，特别是肺部肿瘤，且可减少患者检查次数。

A diagnosis of lung disease by means of digital image combined with fibre bronchoscopy in making selective bronchography and pneumocentesis biopsy

Objective:To investigate the value of diagnosing lung disease by means of digital image combined with fibre bronchoscopy in making selective bronchography and pnermocentesis biopsy.Materials and Methods:First,let 23 patients with such diseases as doubtful lung neoplasm have an X-ray and CT scan examination of the front chest and the lateral chest,and let some of them have a bronchotomogram;then,give them a fibrea bronchoscopy and a brush examination with a pneumocentesis biopsy through fibre bronchoscope under a tele-x-ray.Besides,let them have a selective bronchography by using digital subtractor(DSA),rapid digital photography(8 frame/s),laser photography and 76% compound cardiografin through a fibre bronchoscopy.Results:All cases in this group are examined and diagnosed by this poly technique.They are verified by operative pathology.Among them there are 13 cases with central lung cancer,5 cases with peripheral lung cancer,1 case with bronchiloe pulmonary alveoli cancer,1 case with pulmonary cyst tumour,2 cases with endobronchial tuberculosis and atelectasis and 1 case with inflammatory pseudocarcinoma.Conclusion:This technique is helpful in diagnosing various lung diseases accurately,especially lung neoplasm,and even in reducing the times in examining the patients.
