摘    要:用经颅多普勒超声(TCD)观察15例急性重型颅脑损伤患者颅内压增高后大脑中动脉血流速度(FVmca)变化。同时观察颅内压、血流速度与肱动脉血压的关系。结果发现,随颅内压增高,FVmca平均流速(TAM)和舒张期末流速(Vd)下降,脉动指数(PI)升高。当颅内压接近肱动脉血压的舒张压时,Vd消失为零;等于或略高于舒张压时,Vd呈反向血流。提示用TCD可无创性地推测颅内压增高程度。

Abstract:The transcranial Doppler (TCD),an ultrasonic diagnostic apparatus,was used to measure the ve-locity of the blood flow in the middle cerebral artery (FVmca)in 15 acute severe head injury patientswith raised intracranial pressure. The relation between intracranial pressure(ICP),FVmca and brachialartery pressure was observed simultaneously. The results showed that as the ICP went up,the time av-eraged mean (TAM) and the end--diastolic velocity (Vd) decreased and the pulsatility index (PI) in-creased. The Vd decreased to zero when the ICP increased nearly to the diastolic brachial artery pressure; and complete diastolic retrograde flow velocity was found when the ICP increased to the levelequal to or slightly higher than the diastolic brachial artery pressure. It was indicated that the TCDcould offer a noninvasive evaluation of the degree of increase of the ICP.
