中枢神经系统黑色素瘤的影像诊断

摘    要:目的:分析中枢神经系统黑色素瘤的MRI影像特征。材料和方法:10例中枢神经系统黑色素瘤患者,其中男女各5例,年龄11-59岁。9例病灶位于颅内,其中3例有皮肤黑色素瘤切除史;1例位地颈髓。10例全部行MRI检查,其中4例增强MRI扫描,2例CT平扫。结果:9例MRI均表现为不均-短T1、短T2信号,并在T2WI可见低信号肿块影,另1例显示不均一长T1短T2信号。4例为不均一明显强化,其中1例沿软脑膜转换;2例CT平扫为血肿密度。结论:MRI显示黑色素瘤具有特征性,显示脑膜浸润的范围和程度优于CT。

Abstract:To analyze the MRI features of the central nervous system melanoma. Materials and Methods: 10 cases with central nervous system melanoma were analyzed, which consist of 5 males and 5 females, ranging from 11 to 59 years. 9 cases were intracranial lesions, and among them 3 cases have history of cutaneous melanoma resection. 1 case was located in the cervical spinal cord. All of 10 cases underwent MR examination , and 4 cases accepted contrast - enhanced MR scans; 2 cases underwent plain CT scans. Results: 9 cases demonstrated inhomogeneous short T1 short T2 signal intensity only 1 case demonstrated inhomoge-neous long T1 short T2 signal intensity. All lesions appeared as hypointense mass on T2 WT; After contrast administration, the lesions were enhanced strongly but inhomogeneously, leptomeningeal metastasis was found in 1 case; The 2 cases underwent CT scan demonstrated similar density to the hematoma. Conclusion: The MR appearance of melanoma is characteristic, and MRI is superior to CT in showing the extent and degree of meningeal infiltration.
