恩必普软胶囊治疗急性脑梗死临床疗效观察

摘    要:目的评价恩必普对急性脑梗死临床疗效、安全性及对患者血浆金属基质蛋白酶-9(MMP-9)含量的影响。方法选择2008年1月至2009年5月本院神经内科收治的急性脑梗死患者78例,随机分为治疗组和对照组。两组基础治疗相同,治疗组加用恩必普200 mg,每日3次口服,连用20 d。第21天采用美国国立卫生院神经功能缺损评分(NIHSS)量表进行临床疗效评定。分别在确诊后24 h内和治疗20 d时测定血浆MMP-9含量,另选30名健康体检者检测血浆MMP-9含量。结果治疗组21 d的NIHSS评分与对照组比较,差异有统计学意义(P0.01),两组NIHSS评分治疗后较治疗前差异均有统计学意义(P0.01);恩必普软胶囊治疗疗效优于对照组,治疗组与对照组比较差异有统计学意义(P0.01)。急性脑梗死血浆MMP-9含量明显高于健康对照组,治疗组、对照组与健康对照组比较差异有统计学意义(P0.01);治疗20 d后,治疗组血浆MMP-9下降幅度明显高于对照组,差异有统计学意义(P0.05)。结论恩必普软胶囊可用于急性脑梗死的治疗,能够促进神经功能的恢复,其机制可能与其减轻炎性反应,缩小脑梗死体积有关。

Abstract:Objective To evaluate the curative effect of dl-3-butylphthalide(NBP) on the acute cerebral in farction and its effect on blood plasma MMP 9. Methods 78 cases of patients with acute cerebral infarction were randomly divided into NBP treatment group and conventional treatment group(control group). NIHSS scale was assessed for evaluating the clinical efficacy at the 21 st day. And MMP-9 was selected to measure within 24 hours after diagnosis and at the 20st day after treatment respectively. Results The NIHSS score at the 21st d all in NBP treat ment group had statistical significance compared with that of the control group (P〈0. 01) ;After treatment the NIHSS score had obviously improved in the two groups(P〈0.01);The NBP treatment group was more effective than that of the control group. MMP-9 plasma concentration after acute cerebral infarction was significantly higher than that of the normal group. After the treatment for 20 days by NBP, the MMP 9 was decreased significantly,compared with the control group(P〈0. 05). Conclusion NBP could effectively improve the neurological deficits in patients with acute cerebral infarction and its mechanism may be related with the reduction of the infarct volume.
