Hs-CRP在妊娠合并急性胰腺炎严重性的预测价值及其机制

目的探讨高敏C-反应蛋白在妊娠合并急性胰腺炎严重性的预测价值及其机制。方法收集65例妊娠合并急性胰腺炎患者,依据Ranson评分标准,将患者分为轻型胰腺炎（30例）和重型胰腺炎（35例）,分别于入院时,第3天及第7天检测两组患者外周血白细胞总数、血浆谷丙转氨酶、尿素氮及高敏C-反应蛋白水平。结果重型胰腺炎患者血淀粉酶及脂肪酶均明显高于轻型患者（P〈0.05）;入院时、第3天及第7天,轻型胰腺炎患者外周血白细胞总数、血浆谷丙转氨酶、尿素氮及高敏C-反应蛋白水平均明显低于重型胰腺炎患者（P〈0.05）;血浆高敏C-反应蛋白水平与谷丙转氨酶和尿素氮水平呈正相关,相关系数分别为r=0.927,P〈0.05和r=0.934,P〈0.05。两组患者甘油三酯及总胆固醇水平无明显差别。结论血浆高敏C-反应蛋白水平可以在一定程度上预测妊娠合并急性胰腺炎患者严重程度,其机制可能与血浆Hs-CRP水平能够反应患者肝功能及肾功能损害有关。

Predictive value and mechanism of hs-CRP on the severity of pregnancy with acute pancreatitis

Objective To investigate predictive value and mechanism of hs-CRP on the severity of pregnancy with acute pancreatitis.Methods 65 pregnant patients with acute pancreatitis were enrolled and assigned into mild group（n=30）and severe group（n=35） according to Ranson score system.Number of white blood cell（WBC）of peripheral blood and serum levels of alanine transaminase （ALT）,blood urine nitrogen（BUN）and high-sensitive C reactive protein（hs-CRP）were evaluated at initial administration at the third day and at the seventh day.Results Serum levels of amylase and lipase were significantly higher in severe group than those in mild group（P 〈 0.05）.Number of WBC,serum levels of ALT,BUN and hs-CPR were significantly lower in mild group comparing to severe group（P 〈 0.05）.Serum level of hs-CPR was positive correlated with ALT（r=0.927,P 〈 0.05）and BUN（r=0.934,P 〈 0.05） respectively.There was no difference of triglyceride and total cholesterol between these groups.Conclusion Serum level of hs-CPR has predictive value on the severity of pregnant patients with acute pancreatitis and the mechanism may be ascribed to the association between serum level of hs-CRP and impairment of live and kidney function.
