不孕患者腹腔镜手术中抗生素使用探讨

目的用回顾性方法探讨妇科不孕患者腹腔镜手术中抗生素的合理应用。方法以2002年1月～2008年6月88例不孕患者为研究对象,分为两组,患者均先后有两次手术史,观察组20例于术前30min使用头孢他啶1g,术后使用头孢他啶1g,q12h,共3次;对照组68例为术后使用头孢他啶1g,q12h,共8次;比较两次手术中盆腔情况的变化。结果比较两种不同抗生素的使用方法对盆腔内粘连及输卵管病变的影响,发现两组间无显著性差异。结论围手术期使用抗生素与术后使用抗生素对不孕患者腹腔镜手术后盆腔粘连的再次发生和输卵管病变无影响,抗生素使用时间短,可减少药物耐药性的产生,故应在不孕患者的复孕手术中提倡围手术期抗生素的使用。

Search after Using Peritoneoscope Surgery Antibiotic in Sterile Patient

Objective With review method discussion gynecology department sterile patient peritoneoscope surgery in antibiotic reasonable application.Methods From January 2002 to June 2008,68 women who had performed laparoscopy were divided into 2 groups,2 groups stochastically,2 group of patients have two surgery histories,the observation group uses the single dosage antibiotic in the technique previous 30 points,after the technique,use the antibiotic 3 times,control group after the technique uses 4 day of 8 antibiotic.Pelvic cavity change of situation＇s discusses the sterile patient peritoneoscope surgery after two surgeries in whether to need the long time use antibiotic.Results Two group of different antibiotic＇s application methods,continually and the oviduct pathological change do not have the influence to the pelvic cavity cohesion,compared with latter two groups of non-significance difference.Conclusion After the antibiotic period of revolution long cannot reduce the sterile patient peritoneoscope surgery,the pelvic cavity adhesion＇s occurrence once more,does not have the aggravation to the oviduct pathological change,therefore should,in the sterile patient duplicate is pregnant in the surgery to advocate that encircles the surgery time antibiotic the use.
