大肠癌患者血清VEGF、CEA表达水平与浸润转移关系的研究

目的：探讨大肠癌患者血清VEGF及CEA的表达水平与大肠癌浸润和转移的相关性。方法：应用EusA法检测68例大肠癌患者血清VEGF、CEA的表达水平，根据不同临床病理因素分组，分别比较各组间阳性率的差异；并比较单检与联检VEGF、CEA在诊断大肠癌肝转移中的价值。结果：大肠癌患者血清VEGF的表达水平与患者的性别、年龄无显著相关（P〉0．05），而与肿瘤的组织学分级、浸润深度、淋巴结转移、肝转移及Dukes分期密切相关（P〈0．05或P〈0．01）；CEA的表达水平与患者的性别、年龄、浸润深度及淋巴结转移均无显著相关（P〉0．05），而与肿瘤的组织学分型、肝转移及Dukes分期密切相关（P〈0．05或P〈0．01）。联合检测血清VEGF、CEA对诊断大肠癌肝转移的敏感性可达89．5％。结论：VEGF和CEA与大肠癌的病情发展、浸润和转移有密切相关性，临床上，血清VEGF、CEA的表达水平可作为了解大肠癌生物学行为和判断预后的指标。

The correlation between expression level of serum VEGF and CEA with invasion and metastasis in patients with colorectal carcinoma

Objective:To evaluate the relationship of expression level of serum VEGF and CEA with the development,invasion and metastasis of colorectal carcinoma. Methods:The expression level of serum VEGF and CEA in 68 cases of colorectal carcinoma was detected by ELISA method and its relationship to clinical pathological factor was analyzed. And to evaluate the efficiency of serum VEGF and CEA in diagnosis for liver metastasis of colorectal carcinoma. Results: Serum VEGF positive expression was significantly associated with tumor differentiation, depth of tumor invasion , lymph node metastasis,liver metastasis and Dukes stage(P<0.05 or P<0.01),but its relationship to age and gender was not found(P>0.05); CEA positive expression was significantly associated with tumor differentiation, liver metastasis and Dukes stage(P<0.05 or P<0.01),but its relationship to age,gender,depth of tumor invasion, lymph node metastasis was not found(P>0.05).The diagnostic sensitivity of combination of serum VEGF and CEA is high. Conclusion: VEGF and CEA has close correlation with invasion and metastasis of colorectal carcinoma.The expression level of serum VEGF and CEA is helpful for evaluation of the biological behaviour and outcome of colorectal carcinoma.
