青少年腰椎间盘突出症76例临床探讨

目的:探讨青少年腰椎间盘突出症的发病机理、临床表现、治疗机理和疗效。方法:回顾性分析该病76例临床资料,用手法触诊检查确定腰椎棘突偏歪方向,76例患者全部摄了腰椎正侧位片,全部作了腰椎CT或MRI检查,用冯氏手法治疗了76例患者。结果:本组76例均有棘突偏歪,以L5为最多(33个,55．9％);外伤是造成青少年腰椎间盘突出症的主要原因。青少年和中年人在腰椎间隙变窄发生数、治愈和有效率二组间无明显差异(P>0．05)。青少年组手法治疗有效率98．7％。结论:本病确诊靠MRI和CT检查,冯氏手法是治疗该症的有效方法之一。

Clinical Study of 76 Cases with Adolescent of Protruded Lumbar Intervertebral Disc

Objective: To study the pathogenic mechanism, clinical manifestation and treatment of adolescent of protruded lumbar intervertebral disc (APLID). Methods: Seventy-six cases were retrospectively analyzed with radiographs in A-P and lateral view,MRI or CT scanning on lumbar spine,and the deviation of lumber spinous process assured by special palpation,and 76 patients accepted Feng's Spinal Manipulation (FSM) treatment. Results:Deviation of fifth lumbar spmous process appeared to be the most frequently involved (33 cases 55. 9% ). Trauma are the main cause of APLID. As for incidence of narrowed intervertebral spaces and curative rate of that disease, there is no significant difference between adolescents and the middle-aged patients (p > 0. 05 ) . The effectiverate of manipulation treatment for the adolescent group is 98. 7% (p > 0. 05 ) . Conclusion: A precise diagnosis of the disease is relied mainly on MRI or CT examination. The FSM is an effective choice in the treatment.
