刍拟消异方联合曼月乐用于中重度子宫内膜异位症术后治疗的临床研究

目的寻找一种疗效高、副作用小、复发率低且更经济的治疗子宫内膜异位症的方案。方法选取行保守性手术治疗的中重度子宫内膜异位症患者90例，分为消异方组、曼月乐组及联合组。消异方组为术后3d开始口服消异方，每日1剂．共服用3个月；曼月乐组术后即刻放曼月乐环治疗；联合组采用上述两种方法，在术后即刻放曼月乐环治疗的同时。术后3d开始口服消异方。观察比较治疗前后三组患者的临床总疗效及实验室指标（CA125）的变化。结果联合组的有效率（包括缓解和改善）高于曼月乐组和消异方组，复发率低于两组，差异有统计学意义。三组治疗后CA125水平分别较治疗前显著降低，差异有统计学意义；治疗后曼月乐组和联合组CA125值显著低于消异方组。消异方治疗副反应较少。结论自拟消异方联合曼月乐用于治疗中重度子宫内膜异位症术后治疗能提高临床疗效，降低CA125水平，减少对脂代谢的影响及体重增加的副反应，提高患者的生活质量。

The clinical research on self-prepared Xiaoyifang combining with mirena in treating moderate to severe endometriosis after surgery

Objective To find out a therapy with more effective, safe, economic and lower recurrence in treating En- dometriosis. Methods A total of 90 patients with moderate to severe Endometriosis treated by conservative operation were randomly divided into the Xiaoyifang group, the mirena group and the combined group. The patients in the Xi- aoyi-fang group were given a dose of Xiaoyifang per day for 3 months from 3 days after operation. The patients in the mirena group were laid mirena right after operation. The patients in the combined group were laid mirena fight after operation and given a dose of Xiaoyifang per day for 3 monthes from 3 days after operation. Compare the clinical effi- cacy and laboratory indexes （the level of CA125） before and after treatment. Results The effective rate （including the rate of ease and improve） in the combined group were significantly higher than in the Xiaoyifang and mirena group. The relapse rate in the combined group were significantly lower than the other groups. The level of CA125 in all groups were significantly lower before treatment than after treatment. After treatment, the level of CA125 in the mirena and combined group were significantly lower than these in the Xiaoyifang group. There are less side effects in Xiaoyi- fang. Conclusion Self-prepared Xiaoyifang combining with mirena can improve clinical effect and the quality of patients＇ life. It can reduce the level of CA125, the influence of lipid metabolism and side effects of gain weight.
