改良式泪囊鼻腔吻合术治疗慢性泪囊炎的疗效观察

目的探讨泪囊鼻腔吻合术进行相关改进后治疗慢性泪囊炎的临床效果，为进一步提高慢性泪囊炎手术治疗的成功率提供指导依据。方法收集我院2009年1月～2011年6月期间的住院治疗的慢性泪囊炎患者213例（226只眼），在行泪囊鼻腔吻合术中，仅切断内眦韧带下方部分约1／3，鼻黏膜与泪囊做成相对应的“n”形切口。单纯吻合前瓣后悬吊于骨孔上方骨膜，吻合口处留置导尿管1周。术后观察泪道是否通畅并随访2个月-3年。结果随访2个月以内术眼泪道全部通畅。2个月以上随访观察：痊愈208只眼（92．03％），好转15只眼（6．64％），无效3眼（1．33％）。结论改良式泪囊鼻腔吻合术是一种安全、可靠的手术方式，明显地提高了泪囊鼻腔吻合手术的成功率。

The effect of improved dacryocystorhinostomy for the treatment of chronic dacryocystitis

Objective To investigate the effect of improved daeryocystorhinostomy in the treatment of chronic dacry- ocystitis,to further enhance, the surgical treatment success rate for chronic dacryoeystitis. Methods Collected in our hospital from January 2009 to June 2011,213 cases of hospitalized patients with chronic dacryocystitis （226 eyes）, 213 cases （226 eyes） with chronic dacryocystitis were underwent dacryocystorhinostomy,only cut off 1/3 of the lower part of the medial canthal ligament. Nasal and lacrimal sac made of the corresponding ＂I-I＂ shaped incision,simply anasto- moses rear suspension front flap over the hole in the bone periosteum and detain catheter for 1 week. Lacrimal duct patency were observed and followed two months to three years. Results Within 2 months, all cases had smooth lacrimal passages. After 2 months or more follow-up observation,208 eyes were cured（92.03%）,improved in 15 eyes （6.64%）, only 3 eyes were ineffective （1.33%）. Conclusion Improved dacryocystorhinostomy is a safe and reliable surgical ap- proach, significantly improved the success rate of dacryocystorhinostomv surgical.
