彩色多普勒超声对乳腺良恶性肿块的诊断价值

目的 探讨彩色多普勒超声对乳腺良恶性肿块的诊断价值.方法 收集2011年1月～2013年1月的我院经手术病理证实的乳腺肿块患者60例,观察良恶性肿块的声像图特征以及CDFI比较良恶性肿块的血流分布及血流参数Vmax、Vmin、RI的变化情况.结果 38例良性肿块的二维声像图表现为边缘光滑32例,形态规则30例,有包膜11例,内呈无回声或均质低回声33例,后方回声增强或正常,无组织浸润.其中经病理证实乳腺纤维腺瘤22例,占57.9％.22例乳腺恶性肿块的二维声像图表现为形态不规则19例、边界呈锯齿状、内部呈不均质低回声20例,后方出现声衰减,部分内部可出现微小钙化16例.22例乳腺恶性肿块中,经病理证实浸润性导管癌14例,黏液性腺癌4例,浸润性小叶癌1例,浸润性大汗腺癌l例,乳腺小管癌1例,乳腺腺样囊性癌1例.恶性肿块的Vmax、RI明显高于良性肿块组,差异具有统计学意义（21.7±6.21） vs （18.2±3.9）,（0.72±0.06）vs （0.61±0.08）,P＜0.05].良恶性肿块的Vmin比较无显著差异性,恶性肿块组的血流分级以Ⅲ级为主,良性肿块组以0-I级为主.结论 彩色多普勒超声能清晰显示乳腺良恶性肿块的图像特征,为乳腺肿块的良恶性鉴别提供了有利的参考依据.

Color Doppler ultrasound diagnosis value of benign and malignant breast lumps

Which pathologically confirmed breast fibroadenoma, 22 cases （57,9%）. 22 cases of malignant breast tumor-dimensional sonogram showed 19 cases of ir- regular, jagged border, was not homogeneous hypoechoic internal 20 cases, posterior acoustic attenuation occurs, there may be some internal microcalcifications in 16 cases. 22 cases of malignant breast tumors in pathologically con- firmed invasive ductal carcinoma in 14 cases, 4 cases of mucinous adenocarcinoma, invasive lobular carcinoma, in- vasive adenocarcinoma profusely 1 cases, one case of mammary duct carcinoma, adenoid cystic carcinoma of the breast one case. Malignant tumor Vmax, RI was significantly higher than benign lumps group （21.7 ± 6.21 vs 18.2 ± 3.9,0.72 ± 0.06 vs 0.61 ± 0.08, P 〈0.05）, Benign and malignant tumors of Vmin no significant difference malignant tumor group dominated flow grade to grade III, benign tumor group to 0-I level based. Conclusion Color Doppler ul- trasound can clearly show the benign and malignant breast tumor image features for benign and malignant breast lumps provide a favorable reference.
