胎儿双顶径估计孕龄的方法研究

目的建立基于人群基础的双顶径估计孕龄公式和参照图表,并与国内外其他孕龄估计公式或对照表进行比较。方法研究有效样本18933例,孕龄区间为12～20 w。将双顶径数据和孕龄进行多元线性模型拟合,并使用Z-记分方法将得到的最终双顶径估计孕龄公式与公开发表或正在使用的其他双顶径估计孕龄公式或对照表进行比较分析。结果国内孕龄估计公式或对照表之间差异较大,本文所得孕龄估计公式与内地孕龄估计公式或对照表差异也参差不齐,但与香港孕龄估计公式较为接近,且在各孕周差异校正标准差前提下较为恒定。结论建立了新的基于人群的双顶径估计孕龄的公式,并将之应用于临床和相关研究。

A Research on Approach of Estimating Gestational Age by Fetal Biparietal Diameter

Objective To establish population-based reference equation and chart of estimating gestational age by fetal biparietal diameter(BPD) and compare them with other equations and charts.Methods 18,933 valid samples were studied,gestational ages ranged from 12 to 20 weeks.Multivariate linear model was fitted to BPD and gestational age.Z-scores was used to compare the final reference equation with other gestational age estimated equations or charts on publication or in use.Results Equations or comparison tables were used to estimate gestational age in our country differed greatly.The difference between the equation acquired from this study and the other equations or reference charts in our country was big.But our equation was closer to the one from HongKong,and under the premise of correcting the Standard Deviation of gestational age,the equation was stable.Conclusion New population-based equation of estimating gestational age by BPD was established and could be applied to clinical use and related researches.
