下颌骨动静脉畸形的介入栓塞和手术治疗

目的探讨下颌骨动静脉畸形的根治性治疗方法和经验。方法对3例下颌骨动静脉畸形分别予介入栓塞结合手术切除2例,单纯介入栓塞1例,随访并评价治疗效果。结果随访6~36个月,介入栓塞结合手术切除2例均治愈,保存了下颌骨,骨腔新骨形成,面形及口腔功能恢复良好;单纯介入栓塞的患者,术后3个月牙龈再次大出血,予以切除下颌骨同期修复重建。结论介入栓塞结合手术治疗下颌骨动静脉畸形,可达根治肿瘤又保存功能的目的。

Embolization and surgical treatment of central arteriovenous malformation of mandibular jaw

Objective To explore the methods and experience of radical therapy of central arteriovenous malformation of the jaw.Methods Among 3 cases with central arteriovenous malformations of the mandibular jaw,2 cases were treated by intravascular embolization and curettage,and 1 case was treated by intravascular embolization alone.The patients were followed up and the results of treatment were evaluated.Results After 6-36 months of follow-up,the two patients treated with intravascular embolization and curettage were successfully healed and had no bleeding,and new bone formation was noted,but the other one was bleeding again 3 months later.Conclusion Intravascular embolization and curettage are effective to radical therapy of jaw with arteriovenous malformationa and retain the function of the jaw to the fullest extent,because permanent eradication of varix may preclude reservoir effect.
