C反应蛋白、胱氨酸蛋白酶抑制剂C与慢性心力衰竭左心室射血分数的关系

目的探讨血浆超敏C反应蛋白（hs—CRP）和胱氨酸蛋白酶抑制剂C（cystatin—C）浓度与慢性心力衰竭（CHF）左心室射血分数（LVEF）的关系，总结两者在CHF发展过程中的临床意义。方法选择CHF患者102例，根据二维超声心动图检查结果分为LVEF≤50％病例组（A组）54例和LVEF〉50％病例组（B组）48例，测定血浆hs—CRP和cystatin-C水平，并与30例正常对照组（C组）进行对比分析。结果A组和B组hs—CRP，cystatin—C均较C组明显升高（P〈0．01），且A组hs—CRP较B组明显升高（P〈0．01），但A组cystatin—C与B组比较，差异无统计学意义（P〉0．05）；Cystatin—C与hs—CRP呈正相关性（r=0．248，P〈0．05）。结论CHF随着LVEF值降低，血浆hs—CRP和cystatin—C值升高，两者结合更有利于评价CHF临床严重程度。

Relationship between high sensitivity C-reactive protein, cystatin C and left ventricular ejection fraction in patients with chronic heart failure

Objective To explore the relationship between high sensitivity C-reactive protein （hs-CRP） ,cystatin C and left ventficular ejection fraction（LVEF） in patients with chronic heart failure （CHF）. Method 102 patients with chronic heart failure were divided into the LVEF≤50% group（ group A, n = 54）and LVEF 〉 50% group （group B, n = 48 ）. The plasma hs-CRP and cystatin-C were analyzed in these patients and 30 normal controls（ group C）. Results The plasma hs-CRP and eystatin-C in group A and group B were significantly higher than those in group C （ P 〈 0. 01 ）. Compared with the group B, the concentrations of hs-CRP in group A were significantly higher（ P 〈 0.01 ） , while the mean plasma cystatin-C were slightly higher in group A than group B（ l. 50 mg/L vs 1.34 mg/L） ,but the difference was not statistically significantly（ P 〉 0.05 ）, Elevated hs-CRP is associated with cystatin-C （ r = 0. 248, P 〈 0. 05 ）. Conclusion The plasma hs-CRP and cystatin-C were increased with the lower LVEF in patients with chronic heart failure, and the combined of the two indices are helpful in evaluating the clinical outcomes in patients with chronic heart failure.
