高频振荡通气治疗早产儿重症呼吸衰竭的临床分析

摘    要:目的:评价高频振荡通气对治疗早产儿重症呼吸衰竭的疗效。方法:将52例早产儿重症呼吸衰竭随机分成两组,26例采用常频机械通气(CMV)治疗,另26例采用高频振荡通气(HFOV)治疗。观察两组患儿的肺功能变化,并发症及转归等。结果:HFOV组患儿在治疗6、12、24、48h,氧浓度(FiO2)、二氧化碳分压(PaCO2)、氧合指数(OI)明显下降,a/APO2(动脉/肺胞氧分压比值)明显上升,与治疗前相比差异有统计学意义(P〈0.05)或差异有显著统计学意义(P〈0.01);且与CMV组比差异有统计学意义(P〈0.05);HFOV组患儿上机时间、氧疗时间、病死率及并发症均较CMV组低,差异有统计学意义(P〈0.05)。结论:HFOV能较CMV更好地改善氧合和通气功能,迅速纠正早产儿重症呼吸衰竭,减少上机时间及氧疗时间,提高存活率,疗效较CMV好。

The effects of high frequency oscillatory ventilation in premature infants with severe respiratory failure

Abstract:Objective:To investigate the effects of high frequency oscillation ventilation(HFOV) in premature infants with severe respiratory failure.Methods:52 premature infants with severe respiratory failure were randomly assigned to two groups,26 premature infants were treated with conventional mechanical ventilation(CMV),another 26 premature infants were treated with high frequency oscillation ventilation(HFOV).Results:FiO2,PaCO2,OI decreased significantly and a/AO2 increased significantly in HFOV group after 6,12,24,48 hours ventilation.Be compared with before HFOV treating there was statistical significance(P〈0.05) or(P〈0.01).Be compared with CMV treating group there was statistical significance(P〈0.05);Duration of ventilation in HFOV group was less than CMV group,there was statistical significance(P〈0.05).Survival rates in th HFOV group was higher than CMV group,there was statistical significance(P〈0.05).Conclusion:HFOV can better improve arterial oxygenation and reduced the duration of ventilation in the premature infants with severe respiratory failure than CMV.As well as icreasing survival rates.HFOV is an effective method for premature infants with severe respiratory failure.
