摘    要:目的 :探讨强直性脊柱炎导致肺功能损害的早期变化。方法 :采用美国Sensormedics 6 2 0 0型人体体积描记仪检测 31例强直性脊柱炎患者及 31例健康人的肺功能 ,比较两组之间肺功能各项指标。结果 :2 0例 (6 4 5 % )强直性脊柱炎患者有肺功能改变 ,主要表现为PEF、V50 、V2 5降低 ,分别占 5 1 6 %、32 2 %、4 5 1% ;RAW的升高 ,占 32 2 % ,两组上述指标相比有显著差异 (P <0 0 1,P <0 0 5 ) ,显示AS病人最大呼气中期流速降低及气道阻力升高。结论 :强直性脊柱炎患者导致肺功能的早期改变主要以小气道功能损害为主

Abstract:Objective: Study on the pulmonary function injury caused by ankylosing spondylitis .Methods: Test pulmonary functions (TLC, VC, FEVI%, MVV, PEF, V 50, V 25, DLCO, RAW) of 31 patients with ankylosing spondylitis were measured using U.S.A. Sensormedics 6200 Automated Body Plethysmograph and 31 healthy volunteers were similarly tested and used as controls.Results: Out of 31, 20 (64.5%) patients' pulmonary function indexes TLC, VC, MVV, FEV1%, PEFV 50V 25 and blood-air index PaO 2 decreased, while their DLCO and RAW indexes increased,detailedly,TLC(19.2%),VC(22.5%),MVV(25.7%),FEV1%(3.8%),PEF(51.6%),V 50(34.6%),V 25(45.1%),PaO 2(2.7%),DLCO(3.2%) and RAW(32.2%).Compared with controls,the decreases of PEF,V 50and V 25and the increase of RAW were significant(P<0.01,P<0.05). Conclusion: Ankylosing spondylitis is a sero (rheumatoid factors )-negative arthritis which principally affects the intervertebral joints and sacroiliac joints, however, it can simultaneously incur the pulmonary function injury. The early pulmonary function injury resultant from ankylosing spondylitis is the decrease of small airway function and increase of airway resistance, and the injnry becomes deeper with the disease advancing.
