肺炎患者血清降钙素原和C反应蛋白的表达及临床意义

目的 观察肺炎患者血清中降钙素原（PCT）和C反应蛋白（CRP）的表达情况,并探讨其临床意义.方法 检测113例肺炎患者及40例健康对照人群的血清PCT和CRP表达情况.结果 肺炎患者的血清PCT、CRP阳性检出率和浓度均显著高于对照组（P＜0.05或P＜0.01）,且细菌性肺炎组高于其他2组肺炎患者（P＜0.05或P＜0.01）;血清PCT检测肺炎患者的敏感度、特异度、阳性预测值和阴性预测值分别为51.33％、95.00％、96.67％和40.86％.血清CRP检测肺炎患者的敏感度、特异度、阳性预测值和阴性预测值分别为46.02％、97.50％、98.11％和39.00％.结论 肺炎尤其是细菌性肺炎患者的血清PCT和CRP表达显著上调,二者可作为肺炎诊断及鉴别肺炎类型的参考指标.

Clinical significance and expression of serum procalcitonin and C-reactive protein levels in patients with pneumonia

Objective To explore the expression of serum procalcition （PCT） and C-reactive protein （CRP） in patients with pneumonia. Methods Levels of PCT and CRP of 113 pneumonia patients and 40 healthy people were observed. Results The positive rate and concentration of PCT and CRP in pneumonia patients were significantly higher than those in control group （P 〈 0.05 or P 〈 0.01 ）, and of which was highest in bacterial pneumonia patients （P 〈 0.05 orP 〈 0.01）. The sensitivity, specificity, positive predictive value and negative predictive value of serum PCT detection were 51.33 %, 95.00 %, 96.67 % and 40.86 % respectively. Detection index mentioned above for CRP were 46.02 %, 97.50 %, 98.11% and 39.00 % respectively. Conclusion Patients with pneumonia, especially bacterial pneumonia, have increasing levels of serum PCT and CRP, and which are potential biomarker for the pneumonia diagnosis and identification.
