人工髋关节置换治疗高龄股骨颈骨折

【目的】探讨使用人工股骨头或全髋置换治疗高龄股骨颈骨折的疗效。【方法】回顾2003年6月至2008年12月66例80岁以上股骨颈骨折患者行人3-股骨头或全髋置换术的临床资料。【结果】66例患者患者平均住院15．6d，45例经6个月至5年随访，平均随访时间为2．8年。Harris髋关节评分术后1年平均（78．6±10．3）分，术后2年平均（78．4±10．1）分。【结论】对高龄股骨颈骨折应尽早手术，人工关节置换术可避免股骨头坏死或骨折不愈合，缩短康复时间，极大提高患者的生存率和生活质量。合理的围手术期治疗是成功的关键。

Hip Arthroplasty in the Treatment of Femoral Neck Fractures in Senile Patients

[Objectivel To explore the effect of artificial femoral head replacement （AFHR） and total hip replacement（THR） for senile patients with femoral neck fracture. [Methods]The clinical data of 66 patients （aged over 80） with femoral neck fracture treated with THR or AFHR between June 2003 and December 2008 were analyzed retrospectively. [Results] The average in hospital time of 66 cases was 15.6 days, and 45 cases of them were followed up for 6 months-5 years, with an average of 2.8 years. The average of Harris hip scores at 1 year and 2 years after operation was 78.6±10.3 and 78.4±10.1, respectively. [Conclusion] Aged pa tients with femoral neck fractures should receive surgical treatment as early as possible. Arthroplasty can avoid osteonecrosis of the femoral head and nonunion, and accelerate rehabilitation, therefore greatly improve the survival rate and the quality of life in aged patients with femoral neck fractures. The reasonable perioperative treatment is a key to gain success.
