葛根芩连五炭汤对溃疡性结肠炎患者炎症因子和氧化应激的影响

目的:观察葛根芩连五炭汤治疗溃疡性结肠炎(ulcerative colitis,UC)的临床疗效及其对患者炎症因子、氧化应激的影响。方法:126例UC患者按照随机数字表法平均分为对照组和观察组。对照组给予美沙拉秦;观察组在对照组治疗基础上给予葛根芩连五炭汤。比较两组中医证候疗效,治疗前后Baron内镜评分,氧化应激指标及炎性细胞因子水平,并评价两组用药安全性。结果:治疗后,观察组中医证候有效率为92.1%,对照组有效率为76.2%,两组比较,差异有统计学意义(P0.05)。两组治疗后Baron内镜评分均显著低于治疗前(P0.01);且观察组治疗后Baron内镜评分显著低于对照组同期(P0.01)。与治疗前相比,两组治疗后血清超氧化物歧化酶(superoxide dismutase,SOD)水平均显著升高、丙二醛(malondialdehyde,MDA)水平均显著降低(P0.01);且观察组治疗后血清SOD、MDA水平,均显著低于同期对照组(P0.01)。与治疗前比较,两组治疗后血清肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-6(interleukin-6,IL-6)水平,均显著下降(P0.01),IL-10水平均显著上升(P0.01);且观察组治疗后TNF-α、IL-6均显著低于同期对照组(P0.01),IL-10高于同期对照组(P0.01)。观察组不良反应率为3.2%,对照组为7.9%,两组比较,差异无统计学意义(P0.05)。结论:葛根芩连五炭汤治疗UC疗效显著,能有效减轻患者临床症状,改善结肠黏膜溃疡,调节机体氧化/抗氧化、平衡炎性因子。

The Effect of Gegen Qinlian Wutan Tang on Inflammatory Cytokines and Oxidative Stress of Ulcerative Colitis Patients

Objectives:To explore Gegen Jinlian Wutan Soup clinic effect on ulcerative colitis (UC).Methods:Select 126 cases with UC in our hospital from June 2014 to August 2016 and divide them into two groups randomly.Provide common therapy to all cases.Provide mesalazine to control group additionally.Provide Gegen Jinlian Wutan Soup to observation group.Record and compare two groups TCM syndromes,the score of Baron endoscopy before and after therapy,oxidative stress and inflammatory cytokine levels,evaluate two groups' medicine safety as well.Results:After receiving therapy for two month,observation group's overall TCM syndromes effective rate was 92.1％ which was significant higher than control group's 76.2％ (P ＜ 0.05).Two groups' Barcon endoscopy score after therapy was significant lower than that before therapy(P ＜ 0.01).Observation group's baron endoscopy score after therapy was significant lower than that of control group at same time(P ＜ 0.01).Two groups' SOD level improves significantly and MDA level decrease significantly after therapy if comparing with those before therapy(P ＜ 0.01).Observation group's SOD level and MDA level after therapy were better than those of control group at same time(P ＜ 0.01).Two groups' TNF-α,IL-6 level after therapy decrease significantly than those before therapy(P ＜ 0.01).Two groups' IL-10 level after therapy increases significantly than that before therapy(P ＜ 0.01).Research group's inflammatory cytokine levels after therapy improved significantly than that of control group at same time(P ＜ 0.01).Here was no Statistical significance if comparing observation group's adverse reaction rate 3.2％ with control group's 7.9％ (P ＞0.05).Conclusion:The application of Gegen Jinlian Wutan Soup can help to relieve patient's clinic symptom,improve the colonic ulcer,regulate body oxidation/anti-oxidation,promote/anti-inflammatory factor balance to delay disease progression,it is safe with good clinic effect.It brings new mode and idea for UC's clinical prevention and therapy.
