六味地黄汤合四物汤治疗类风湿关节炎临床研究

目的:观察中药六味地黄汤合四物汤治疗类风湿关节炎的临床疗效。方法:120例类风湿关节炎患者按照随机数字表法分为观察组和对照组,每组60例。对照组给予甲氨蝶呤片(每次10 mg,每周1次),观察组给予六味地黄汤合四物汤(水煎服,每日1剂,分早晚各服用一次)。3个月后,比较两组治疗前后血清I型胶原C端肽酶(CTX-I)、抗CCP抗体、类风湿因子(rheumatoid factors,RF)、免疫球蛋白、炎症因子和临床症状。结果:两组患者治疗后白细胞介素-10(interleukin-10,IL-10)、IL-1和肿瘤坏死因子-α(tumour necrosis factor-α,TNF-α)、血沉、C反应蛋白(C reactive protein,CRP)、RF、CTX-I、抗CCP抗体、Ig M和Ig G均明显改善(P0.01),但观察组治疗后以上指标改善效果明显优于对照组(P0.01)。两组患者治疗后关节僵硬、关节疼痛、日常活动以及综合评分均明显改善(P0.01),但观察组临床效果明显优于对照组(P0.01)。结论:六味地黄汤合四物汤治疗类风湿关节炎临床疗效显著,可通过抑制免疫球蛋白合成、炎症因子以及类风湿因子等改善类风湿关节炎患者病情。

Clinical Research on Liuwei Dihuang Tang and Siwu Tang Treatment of Rheumatoid Arthritis

Objective:To analyze the clinical effect of Liuwei Dihuang Tang and Siwu Tang treatment of rheumatoid arthritis.Methods:120 patients with rheumatoid arthritis were randomly divided into two groups,each of 60 cases.The control group was given oral methotrexate tablets (10mg per time,one time each week),and the observation group was treated with Liuweidihuang and Siwutang(water decoction,daily 1 agent,orally administration in the morning and night).Both groups were given continuous treatment for 3 months,observed and compared before and after treatment of serum C-telopeptide of type I collagen enzyme (CTX-I),antiCCP antibodies,rheumatoid factor (RF),immunoglobulin associated inflammatory factors and clinical symptoms.Results:After treatment,patients with inflammatory cytokines IL-10,IL-1 and TNF-α,ESR,CRP,RF,CTX-I,anti-CCP antibodies,IgM and IgG were significantly improved (P ＜ 0.01),but the observation group after treatment,improve inflammatory factors,rheumatoid factor,and clinical effects of immune proteins indicators than the control group(P ＜ 0.01),there was a significant statistical significance.After treatment,joint stiffness,joint pain,daily activities and comprehensive score of the two groups were significantly improved (P ＜ 0.01),but the clinical effect of observation group better than the control group(P ＜ 0.01).Conclusion:Liuwei Dihuang Tang and Siwu Tang can improve condition of patients with rheumatoid arthritis by inhibiting the synthesis of immunoglobulins,inflammatory cytokines and rheumatoid factor,and Liuwei Dihuang Tang and Siwu Tang can effectively treat rheumatoid arthritis.
