健脾补精汤对胃癌术后脾虚证患者临床疗效及免疫功能的影响

目的:探讨健脾补精汤对胃癌术后脾虚证患者临床疗效及免疫功能的影响。方法:选取2014年1月—2015年11月行胃癌手术的患者92例,采用随机数字表法分为对照组与研究组,对照组患者术后早期给予胃肠营养干预,研究组患者在对照组基础上联合健脾补精汤治疗,观察治疗后两组患者中医证候积分、治疗效果及相关免疫指标水平。结果:治疗后两组患者中医证候积分均有明显改善,其中研究组患者神疲乏力、纳呆消瘦、面色萎黄、便溏等证候积分明显低于对照组(P0.05),而腹泻、呕吐两组比较差异无统计学意义(P0.05);研究组患者治疗后免疫球蛋白IgA、IgM、IgG、血浆胃泌素水平均较对照组明显升高,差异有统计学意义(P0.05);研究组患者治疗后CD4~+、CD4~+/CD8~+升高、CD8+降低,与对照组比较,差异有统计学意义(P0.05);研究组患者治疗后排气时间、排便时间、置管时间短于对照组,总引流量少于对照组,差异有统计学意义(P0.05)。结论:早期胃肠营养干预基础上应用健脾补精汤治疗胃癌术后脾虚证患者效果较好,可有效改善患者胃肠及免疫功能。

Influence of Invigorating Spleen and Supplementing Essence Decoction on Immune Function of Postoperative Patients of Spleen Deficiency Syndrome with Gastric Cancer

Objective:To explore the influence of invigorating spleen and supplementing essence decoction on immune function of postoperative patients of spleen deficiency syndrome with gastric cancer.Methods:92 cases of patients treated with gastric cancer operation in our hospital from January 2014 to November 2015 were selected,and were randomly divided into control group and study group.The control group were given early parenteral nutrition intervention after operation,and the study group were treated with invigorating spleen and supplementing essence decoction on the basis of the control group.TCM syndrome scores,treatment effect and related immune index were analyzed and contrasted in the two groups.Results:After treatment,lassitude,anorexia and weight loss,sallow complexion,loose stools,diarrhea symptom scores significantly improved in the two groups,and lassitude,anorexia and weight loss,sallow complexion,loose stools and other symptoms in the study group were significantly lower than those of the control group (P ＜ 0.05),while there were no significant differences in diarrhea and vomiting between the two groups (P ＜ 0.05).Immune globulin IgA,IgM,IgG,plasma gastrin levels in the study group after treatment were significantly higher than the control group,the difference was statistically significant (P ＜ 0.05);CD4 +,CD4 +/CD8 + increased,and CD8 + decreased in study group after treatment,and there was statistically significant compared with those of the control group(P ＜ 0.05).There were significant differences in exhaust time,defecation time,intubation time and total induced flow between the two groups (P ＜ 0.05).Conclusion:The effect is better to apply invigorating spleen and supplementing essence decoction on the basis of early parenteral nutrition intervention on postoperative patients of spleen deficiency syndrome with gastric cancer,can improve the gastrointestinal and immune function obviously.
