紫丹银屑胶囊联合窄谱中波紫外线(NB-UVB)治疗寻常型银屑病血虚风燥证临床研究

目的:观察紫丹银屑胶囊联合窄谱中波紫外线(NB-UVB)治疗寻常型银屑病的临床疗效以及对氧化应激的影响。方法:将82例银屑病患者按数字表法随机分为观察组和对照组,每组41例。对照组采用波长为311nm的NB-UVB治疗,每周3次;观察组在对照组基础上口服紫丹银屑胶囊治疗,每次4粒,每天3次。两组患者均给予12周治疗。观察两组患者银屑病皮损程度评分(psoriasis area and severity index,PASI)、临床疗效、血清超氧化物歧化酶(superoxide dismutase,SOD)和谷胱甘肽过氧化物酶(glutathione peroxidase,GSH-Px)水平和不良反应情况。结果:观察组有效率为87.80%,对照组有效率为65.85%,两组比较,差异有统计学意义(P0.05);治疗后4周、8周和12周,观察组患者PASI评分均显著低于同时间点对照组,而血清SOD和GSH-Px水平明显高于同期对照组,差异均有统计学意义(P0.01)。观察组患者的不良反应发生率为0.00%,对照组为19.51%,差异有统计学意义(P0.05)。结论:紫丹银屑胶囊联合NB-UVB治疗寻常型银屑病血虚风燥证可明显降低PASI评分,提高临床疗效及减少不良反应发生率,上调血清SOD和GSH-Px水平可能是其发挥治疗作用的机制。

Clinical Study on Zidan Yinxie Capsule Combined with Narrow Band Ultraviolet in Treating Psoriasis Vulgaris of Syndrome of Wind and Dryness Due to Blood Deficiency

Objective:To discuss the therapeutic efficacy of Zidan Yinxie capsule combined with narrow band ultraviolet(NB-UVB) in treating with psoriasis vulgaris and its influence on oxidative stress.Method:82 patients with psoriasis vulgaris in our hospital of department of dermatology were randomly divided into control group and observation group of 41 patients each by random number table.For control group,patients were treated by NB-UVB with 311 nm wavelength of radiation,3 timets per week.Based on the treatment of control group,patients in observation group accepted Zidan Yinxie capsule by oral with 4 capsules pertime and three times a day.Course of treatment were 12 weeks for two groups.The scores of psoriasis area and severity index(PASI) were compared between both groups.Efficacy and incidence were analyzed between two groups.Serum levels of total superoxide dismutase (SOD) and glutathion peroxidase (GSH-Px) were detected between two groups.Results:The markedly effective rate of treatment group was 87.80％ and superior to control group as 65.85％ with statistical significances (P ＜ 0.05).After 4 weeks, 8 weeks and 12 weeks treatment,scores of PASI in treatment group were evidently lower than that of control group at the same time (P ＜ 0.01).Incidence rate of untoward effect of observation group was 0.00％,which was lower than control group as 19.51％ (P ＜ 0.05).After 4 weeks,8 weeks and 12 weeks treatment,serum SOD and GSH-PX were obviously higher than those of control one with statistical signifieanees(P ＜ 0.01).Conelusion:Zidan Yinxie capsule combined with NB-UVB in treating with psoriasis vulgaris of syndrome of wind and dryness due to blood deficiency could obviously decrease scores of PASI,improve therapeutic efficacy,and decrease incidence of untoward effect.Its mechanism may be related with up-regulating SOD and GSH-Px.
