诱导型一氧化氮合酶在口腔鳞癌中的表达及其意义

摘    要:[目的]探讨口腔鳞癌(OSCC)诱导型一氧化氮合酶(iNOS)的表达与OSCC生物学行为之间的关系。[方法]采用免疫组化法检测10例正常口腔黏膜、17例口腔乳头状瘤、32例鳞状细胞癌组织中iNOS的表达。[结果]①正常121腔黏膜组织不表达iNOS,口腔乳头状瘤和鳞癌中iNOS表达阳性率分别为58.82%和78.12%,三组间比较差异有显著性(P〈0.001)。②高、中、低分化口腔鳞癌组织中iNOS表达阳性率分别是61.54%、88.89%、90.0%(P〉0.05);iNOS表达与口腔鳞癌的病理分级呈正相关(r=0.3)。③iNOS表达阳性率在Ⅲ-Ⅳ期组、有颈淋巴结转移组中分别明显高于Ⅰ~Ⅱ期组、无颈淋巴结转移组(P〈0.05)。④iNOS阴性表达的口腔鳞癌患者3年生存率(83.33%)明显高于阳性者的27.27%(P=-0.022)。[结论]iNOS产生的一氧化氮(NO)可能在口腔鳞癌的发生、发展中起重要作用;口腔鳞癌组织中iNOS表达与其病理分级、TNM分期、颈淋巴结转移及预后关系密切。

Abstract:[Purpose] To investigate the expression of inducible nitric oxide synthase (iNOS) in oral squamous cell carcinoma (OSCC) and the relationship between the expression and biological behaviors of OSCC. [Methods ] Immunohistochemical technique was used to detect the iNOS expression in 10 cases with normal oral mucosa, 17 cases with oral papiloma and 32 cases with OS- CC. [Results ]①The expression of iNOS was negative in nomal oral mucosa tissue. The positive expression rate of iNOS in oral papiloma and in OSCC was 58.82% and 78.12% respectively. The expression rate among these three groups was significantly different (P〈0.001). ②The positive iN- OS expression was 61.54%, 88.89% and 90.0% in high, moderate and low degree differentiation respectively(P〉0.05). Positive correlation was found between expression of iNOS and pathological grade of OSCC with coefficient of 0.3. ③The expression rate of iNOS in stage Ⅲ, Ⅳ and positive neck lymph node groups was significantly higher than that in stage Ⅰ , Ⅱ, and negative neck lymph node groups respectively(P〈0.05). ④The 3-year survival in OSCC patients with iNOS nega- tive was 83.33%, which was significantly higher than that in patients with iNOS positive(27.27%) (P=0.022). [Conclusion] iNOS produced NO might play an important role in the pathogenesis and development of OSCC. The expression of iNOS has closely relaed with pathological grade, TNM stage, neck lymph node metastasis and prognosis in OSCC.
