股前外侧脂肪筋膜瓣游离移植矫治半侧颜面萎缩

目的:探讨以股前外侧脂肪筋膜瓣游离移植为主的半侧颜面萎缩综合矫治。方法:1996年10月～2004年4月,应用以吻合血管的股前外侧脂肪筋膜瓣游离移植为主,加以Medpor植入及二期手术修整,综合治疗半侧颜面萎缩24例。结果:24例组织瓣全部成活,8例同时植入Medpor修复硬组织,7例术后半年行组织瓣修薄和周边凹陷游离脂肪注射移植充填。随访6月～7年,面部轮廓形态满意,双侧基本对称。结论:吻合血管的股前外侧脂肪筋膜瓣可提供充足的组织量、存活率高、供区隐蔽、不牺牲重要血管,辅以相应的非生物材料及治疗手段,可使半侧颜面萎缩得到满意矫治。

Correction of Hemifacial Atrophy Using Free Anterolateral Thigh Adipofascial Flap

Objective: To investigate the cotemporary comprehensive treatment to hemifacial atrophy with the free anterolateral thigh adipofascial flap as the main measure. Methods: From October 1996 to February 2004, a total of 24 patients suffering from hemifacial atrophy were corrected with microvascular anastomotic free anterolateral thigh adipofascial flap and other additional measures according to the symptoms of the deformities. Results: All the flaps survived, the skeleton was reconstructed with Medpor in 8 cases, liposuction of the flaps and fat injection were used in 7 patients 6 months postoperatively. Following up ranged from six months to seven years, the face is symmetry, all patients obtained satisfactory results. Conclusion: The anterolateral thigh adipofascial flap provides adequate tissue, easy to survive, no important artery sacrificed and the donor scar is more easily hidden, it can be successfully used to correct the deformity. Other auxiliary methods are also necessary to gain perfect results of the comprehensive treatment.
