重症监护病房鲍曼不动杆菌院内感染原因的研究

摘    要:目的研究重症监护病房鲍曼不动杆菌院内感染的原因,为制定防控对策提供依据。方法通过环境标本细菌分离鉴定,对重症监护病房物体表面与住院病人带菌情况进行调查与分析。结果在该医院重症监护病房内常接触的物体表面、医务人员手等表面均检出鲍曼不动杆菌;从该病房5例感染病人痰标本中均检出鲍曼不动杆菌。病人感染的鲍曼不动杆菌对头孢他啶、阿米卡星、亚胺培南等9种临床常用抗菌药物均耐药。经采取消毒隔离措施和控制抗菌药物使用之后,该病房内各监测对象均未检出鲍曼不动杆菌,也没有发生新的感染病例。结论该重症监护病房住院病人感染的鲍曼不动杆菌与环境物体表面和医务人员手带菌有相关性,所采取的防控措施有效。

Abstract:Objective To identify the reasons for nosocomial infection of Acinetobacter baumannii in ICU.Methods Bacteria isolation and identification of surrounding samples was used to survey and analyze bacteria carriage of object surface and inpatients in ICU.Results Acinetobacter baumannii were detected from object surface,hands of medical staff and 5 cases phlegm samples of infectious patients in ICU.Acinetobacter baumannii detected from inpatients were resistant to 9 clinical used antibacterial such as Ceftazidime,Amikacin and Imipenem.After adoption of disinfection and isolation measures and usage of controlling antibacterials Acinetobacter baumannii was not detected from monitoring object in patients' rooms and no new infectious case happened.Conclusion The Acinetobacter baumannii nosocomial infections of inpatients in ICU are related with bacteria carriage of surrounding object surface and medical staff' hands and the preventive and control measures are effective.
