探讨抗CCP抗体在类风湿关节炎诊断中的临床价值

目的 探讨抗环瓜氨酸肽抗体（抗CCP抗体）在类风湿关节炎（rheumatoid arthritis,RA）诊断中的临床价值.方法 收集2011年1月至12月在解放军四五二医院住院的RA患者150例设为实验组,选择同期在该医院做体检的健康人员150例设为对照组,检测其抗CCP抗体,同时检测类风湿关节炎常用相关诊断指标类风湿因子（RF）和抗角蛋白抗体（抗AKA抗体）,并对结果进行统计分析.结果 灵敏度、特异性检验,抗CCP抗体、RF和抗AKA的灵敏度分别为78.6％、85.3％和55.3％,特异性分别为96.8％、85.3％和94.7％;kappa-致性分析,以临床诊断作为金标准,抗CCP抗体、RF和抗AKA的kappa系数分别为0.75、0.71和0.50.结论 抗CCP抗体对诊断RA具有较高的灵敏度和特异性,是辅助诊断RA的良好指标.

To explore the clinical significance of the RA by anti-CCP antibody testing

Objective To explore the clinical significance of the RA by anti-CCP antibody testing.Methods Choo sing 150 cases of patients with RA for the experimental group and 150 cases healthy check up for the control group,testing the anti-CCP antibody,RF and Anti-AKA antibody,analyze the results comprehensively.Results Compared of the sensitivity and specificity,the sensitivity of the anti-CCP antibody,RF and Anti-AKA antibody is 78.6％,85.3％,55.3％ and the specificity is 96.8％,85.3％,94.7％.Compared of the kappa,the kappa of the anti-CCP antibody,RF and Anti-AKA antibody is 0.75,0.71,0.50.Conclusion The anti-CCP antibody is a good indexes for the RA diagnosis which has high sensitivity and specificity.
