两种方法治疗HIV阳性结核性胸腔积液的疗效与安全性比较

目的比较2种不同的方法治疗HIV阳性结核性胸腔积液的临床疗效及安全性。方法治疗组26例予中心静脉导管胸腔置管闭式引流术；对照组28例应用传统胸膜腔穿刺抽液术，每周抽液二三次。观察胸水吸收时间、并发症、胸穿次数、医务人员职业暴露、病人住院天数等情况。结果治疗组：胸水吸收时间（10．35±3．02）d，胸穿次数26次，并发症2例，医务人员职业暴露1例，病人住院天数（19．88±3．51）d；对照组：胸水吸收时间（23．89±4．01）d，胸穿次数86次，并发症9例，医务人员职业暴露2例，病人住院天数（31．55±7．96）d。结论中心静脉导管胸腔置管闭式引流术在治疗HIV阳性结核性胸腔积液患者中疗效与安全性方面优于传统胸膜腔穿刺抽液术。

Comparison on Efficacy and Safety of Two Methods in the Treatment of Tuberculous Pleural Effusion Combined with HIV Infection

Objective ] To compare the clinical efficacy and safety of two different methods in the treatment of tuberculous pleural effusion combined with HIV infection. Methods]26 cases in the treatment group were treated by closed thoracic drainage with cen- tral venous catheter, 28 cases in the control group received traditional thoracentesis, 2 ～ 3 times a week. The absorption time of pleural effusion, complication, times of thoraeentesis, occupational exposure of medical staff and hospitalization days were observe. Results ] In the treatment group, the absorption time of pleural effusion was 10.35± 3.02 ） d, there were 26 times of thoracentesis, 2 cases of complication, 1 case of occupational exposure, and hospitalization days was （ 19.88±3.51 ） d. In the control group, the absorption time of pleural effusion was （23.89±4.01 ） d, there were 86times of thoracentesis, 9 cases of complication, 1 case of occupational exposure, and hospitalization days was （ 31.55 ± 7.96 ） d. Conclusion ] The clinical efficacy and safety of closed thoracic drainage with central venous catheter is better than those of traditional thoracentesis in the treatment of tuberculous pleural effusion combined with HIV infection.
